Day Shift
Full Time
Functions:
Case manager for pediatric patients regardless of location.
Coordinate care with team members involved in the care of pediatric patients.
Respond to referrals and complete admissions for pediatric patients.
Participate in further defining and developing clinical documentation tools and patient/family teaching materials.
Participate in providing education internally and externally.
Collaborate with the liaisons to develop and enhance relationships with referral sources.
Act as a resource for referral sources and staff.
Provide care to adult patients as needed
Qualifications:
PA Nursing License required
Minimum of 1 year of recent RN hospice or pediatric experience required
