GENERAL DESCRIPTION

Care Coordinator reports to the department area manager and provides discharge planning services, psychosocial evaluation, counseling and support to clients admitted to the Crisis program area.  The individual works in collaboration with other department area staff to provide a full array of Behavioral Health support.

CHARACTERISTIC DUTIES

1.    Assists with preparing an initial screening, evaluation and psych-social assessment for clients; documents assessment within client record in a timely manner.
2.    Establishes contact with appropriate individuals from client’s support system.
3.    Facilitates groups, individual and family meetings.
4.    Monitors clients to provide ongoing assessment, support and motivational interviewing.
5.    Attends team meetings and actively participates in the development of the multi-disciplinary treatment for each client, including formulation of appropriate and measurable goals.
6.    Assist in the development and participates in programming and activities for clients.
7.    Obtains and/or provides transportation for clients.
8.    Maintains accurate and timely documentation of all services provided in accordance with department standards and regulations.
9.    Begins discharge planning upon admission, identifying potential barriers, disposition options and appropriate community resources in maintaining positive health after discharge.
10.    Develops and maintains awareness of broad spectrum of community resources and knowledge of how to access them as needed by patients.
11.    Provides referrals to BMC and community resources as appropriate.
12.    Pre-certifies psychiatric admissions with insurance companies as needed.
13.    Works within their discipline specifics, performance standards and code of ethics.
14.    Performs other duties as assigned.

PERFORMANCE STANDARDS

1.    Demonstrates flexibility to adapt to changes in workload, schedule, department function and procedures.
2.    Participates actively in creating a positive and harmonious work environment by demonstrating team work and cooperative, responsive behaviors.
3.    Works collaboratively with the client and other behavioral health staff to formulate goals for treatment and effectively engages in discharge planning.
4.    Demonstrates effective communication methods and skills.
5.    Meets/exceeds competency criteria specific to the tasks and duties of the position.
6.    Demonstrates service excellence in all activities and interactions.
7.    Documents assessments on all evaluations
8.    Ability to provide back-up to other Community Crisis department positions as needed.
9.    Works collaboratively with clients and other staff in formulating goals for treatment.
10.    Clients are able to remain outside of hospital inpatient services.

MINIMUM QUALIFICATIONS

1.    Bachelor’s degree in human services field from an accredited educational program.
2.    2+ years working with individuals with chronic mental illness.
3.    Ability to work cooperatively with the multi-disciplinary team.
4.    Excellent written and verbal communication skills
5.    Ability to verbally and physically de-escalate a client utilizing BMC trained programs
6.    Ability to complete electronic charting and documentation
7.    Valid Iowa Driver’s License with ability to obtain and maintain Iowa Chauffer’s license within one month of hire with CSC

PREFERRED QUALIFICATIONS

1.    Experience in the treatment of chemically dependent persons.
2.    Experience with insurance authorization and managed care.
3.    Experience working with law enforcement.  
4.    Meditech experience
5.    Bi-lingual.
6.    Knowledge of community resource and referral options
