We are currently seeking a Person-Centered Plan (PCP) Specialist to join our team! In this role, you will be responsible for supporting the coordination of services for up to 75-80 people within our Washington/Frederick and Western Maryland counties. The PCP Specialist facilitates the development of service plans that reflect individual’s unique strengths, needs, preferences, and aspirations. The PCP Specialist works to ensure the timely completion of all PCPs on their assignment list. As well as works closely with regional teams to review usage of services as they create and revise plans as needed to match trends, and needs of people served. This position is also expected to support the overall operation of Abilities Network when needed. Certain tasks may be asked that do not fall into the essential functions of this job.

This position is primarily remote but will require occasional travel to in-person meetings.
