BlueDot Cares, a premier home care provider dedicated to delivering compassionate and personalized healthcare services, is actively seeking a PRN Nurse to join our team at our new Chapel Hill, NC location. As a PRN Nurse at BlueDot Cares, you will play a vital role in delivering high-quality care to our clients in the comfort of their homes.
Client Assessment and Care Planning
- Conduct comprehensive assessments of clients' healthcare needs, collaborating with clients and their families to develop personalized care plans.
- Create and update care plans based on the evolving needs of clients.
- Perform 90 day followups on all active clients.
Caregiver Orientation and Competency Evaluation
- Participate in new caregiver orientation.
- Help evaluate caregiver competencies.
Monitoring and Observation
- Monitor clients' health status, recognizing any changes and promptly reporting concerns to the healthcare team.
- Provide ongoing observation to ensure a safe and comfortable home environment.
Education and Support
- Educate clients and their families on health management, preventive care, and self-care techniques.
- Offer emotional support and guidance to clients and families facing health challenges.
Communication
- Maintain clear and effective communication with clients, their families, and other healthcare team members.
- Document all care provided accurately and in a timely manner.
Collaboration
- Collaborate with other healthcare professionals, including therapists and care coordinators, to ensure comprehensive and coordinated care.
- Participate in team meetings and contribute insights to enhance overall care quality.
Requirements
- Current and valid nursing license in North Carolina.
- Minimum of 1–2 years of nursing experience, with a focus on home care or a related healthcare setting. Home care experience preferred.
- Strong clinical skills and the ability to adapt to a variety of patient needs.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a collaborative healthcare team.
