What You Will Do
As the RN Care Coordinator-Internal Medicine, you will support the coordination of care for adult patients at IHI. Responsibilities include assisting with intake, care coordination, and providing education and communication with patients, families, and healthcare providers. The RNCC also supports standard clinical workflows, patient tracking, national surveillance activities, data capture, and disease-specific interventions aimed at improving patient outcomes. This role works closely with the CVD physician and CVD APP team to ensure coordinated, high-quality care.
The Opportunity
- Facilitate timely communication of nursing assessments and recommendations with providers, care team members, patients, and families to support comprehensive medical and surgical care planning.
- Coordinate management and tracking of disease-specific care standards, including participation in national surveillance and outcomes programs.
- Review and analyze patient care trends, gaps, and needs to recommend and help implement initiatives that improve patient services and outcomes.
- Identify potentially at-risk family members of affected patients and assist with coordination of appropriate screening or genetic testing.
- Develop and provide culturally competent education for patients, families, and staff using health literacy principles and plain-language guidelines.
- Represent the organization at patient and provider conferences, educational programs, and outreach events.
Requirements
- Completion of an approved nursing program
- Minimum 2 years of experience working with patients with a chronic disease
- Licensed as a registered nurse (RN) in the state of Indiana.
- Valid Indiana Driver’s license and automobile insurance
This position is open to residents of the State of Indiana, and requires routine presence at our Center in Indianapolis, IN. Frequency of on-site presence is dependent upon role and department needs.
