For over 125 years, Calvary Hospital and Calvary @Home located in the Bronx, New York has been dedicated to the palliative care of advanced cancer and other terminally ill patients and their families. Through a caring, compassionate, philosophy of non-abandonment, the hospice provides a full spectrum of palliative services, including emotional, spiritual, and physical care. Maintains patient medical records in Hospice.  Ensures accurate and timely filing into processing patient medical assistant.  Enters data into the Hospice computer system.

ESSENTIAL JOB FUNCTIONS:

  • 1. Programs phone to Answering Service at end of workday.
  • 2. Answers department telephones, relays messages clearly, and responds to inquiries in a timely and appropriate manner.
  • 3. Accurately, sorts and files records and reports in the Patient Medical Record (active and discharge records).
  • 4. Collaborates with LHCSA (HHA) partner agencies to coordinate and secure appropriate Home              Health Aide placements for current patients, ensuring timely coverage and continuity of care.
  • 5. Timely notifies all required providers of a patient’s death, including HHA agencies, DME vendors, referral sources, and interdisciplinary team members, in accordance with established procedures.
  • 6. Orders supplies/equipment for patients assigned.  Requests pick up of rented equipment with vendors for patients discharge as needed.  
  • 7. Verifies that HHA invoices are accurately entered into the system and ensures all reported hours are correct prior to submission.
  • 8. Makes copies and retrieves documents upon request.
  • 9. Works within legal, ethical practice standards to his/her position as well as Calvary’s Code of Conduct.  Complies with appropriate policies and procedures and brings potential compliance matters to supervisor or compliance officer.

Requirements

  •  High School Diploma required. Some college preferred.
  •  Minimum of one year of clerical experience in an office environment is required.
  •  Competency with computer software and peripherals is required.
