As a Licensed Practical Nurse  your work has real weight. You’ll serve older adults in Allegheny County—not in a hospital, but in their homes, communities, and at our Day Health Centers. You’ll be the face they trust and the steady hand they rely on.

• Provide hands-on personal and clinical care (wound care, medication administration, respiratory support)
• Monitor vital signs and report changes to the RN or IDT
• Assist with medication box preparation and physician order follow-up
• Maintain accurate documentation and communication
• Help Participants remain safe and supported in their homes

Requirements

• Active PA LPN license
• Minimum one year of experience  with the elderly and or frail.
• CPR certification
• Valid driver’s license, reliable  vehicle, and auto insurance
