Responsibilities
- Assist individuals with daily living activities such as grooming, personal hygiene, and meal preparation.
- Implement individualized support plans to promote personal growth and development.
- Encourage and facilitate community integration by accompanying individuals on outings and social activities.
- Monitor and document the progress and challenges faced by individuals in their support plan.
- Provide emotional support and encouragement to individuals and their families.
- Maintain a safe and clean living environment for those you support.
- Collaborate with interdisciplinary teams to ensure comprehensive care and effective communication.
Requirements
- High school diploma or equivalent; additional education in social work, psychology, or a related field is a plus.
- Previous experience working with individuals with disabilities is preferred.
- Strong interpersonal and communication skills to effectively interact with individuals and their families.
- Ability to maintain confidentiality and adhere to HIPAA regulations.
- Valid driver's license and reliable transportation for community outings.
- Flexibility to work various shifts, including evenings, weekends, and holidays as required.
- Certification in CPR and First Aid, or willingness to obtain upon hiring.
