At Parx Home Health Care, we believe in being Your Partner at All Times. We are seeking a Clinical Intake & Care Transition Nurse to serve as a trusted clinical partner throughout the patient journey—from referral intake through admission and post-discharge follow-up.
This role is ideal for a nurse who enjoys clinical decision-making, collaboration, patient engagement, and ensuring the right care, at the right time, by the right team.
What You’ll Do
- Serve as the clinical authority reviewing all incoming home health referrals
- Evaluate referrals for clinical appropriateness, completeness, and feasibility
- Proactively obtain missing documentation, orders, or information from physicians and referral sources
- Assess patient needs against staffing models and clinical resources to ensure safe care delivery
- Appropriately accept or decline referrals based on clinical capability and business alignment
- Partner with operations, clinical leadership, and community liaisons to support strategic accounts
- Confirm and coordinate signing physicians for Plans of Care (485s)
- Act as the first clinical point of contact for patients through structured welcome calls
- Educate patients and families on home health services, expectations, and next steps
- Serve as the final point of contact post-discharge, conducting follow-up and satisfaction calls
- Identify unresolved needs, facilitate re-admissions when appropriate, and ensure seamless transitions
- Maintain accurate, timely documentation and ensure compliance with payer, regulatory, and Parx policies
- Contribute to QAPI initiatives by identifying trends, gaps, and opportunities for improvement
Requirements
What We’re Looking For
- Current LPN or (RN) license in good standing
- Home health, intake, care coordination, or transitional care experience preferred
- Strong clinical judgment and ability to balance patient needs with operational realities
- Excellent communication, organization, and relationship-building skills
- Comfortable working in a fast-paced, collaborative environment
- EMR experience preferred (KanTime a plus)
Work Environment
- Monday–Friday schedule with occasional flexibility
- Office-based
- Ability to sit and work at a computer for extended periods
- Occasional travel as needed
- Moderate risk exposure
