Wellness Equity Alliance (WEA) is a novel national public health organization comprised of a multidisciplinary team of population and public health experts with backgrounds in infectious disease, public health, emergency medicine, primary care, cardiology, pediatrics, psychiatry, community health work (CHW), nursing and advanced practice pharmacy. We work nearly exclusively with underrepresented communities, fundamentally addressing health-care disparities and the social determinants of health (SDoH) that have been amplified during the COVID-19 pandemic, prioritizing the following:

WEA is actively seeking compassionate and driven individuals for several pivotal roles in our groundbreaking School-Based Health Center (SBHC) program for Bernalillo County, New Mexico. Our approach will focus on health promotion, prevention, and treatment centered on behavioral health and primary care (including sexual health and gender-affirming care) located in large mobile units (including a brick-and-mortar clinic) in schools across the county. JOB DESCRIPTION


Purpose of the position:

The Clinical Case Manager plays a vital role in delivering comprehensive, compassionate, and accessible behavioral health and case management services to individuals and families across the Los Angeles and Inland Empire regions. This position focuses on supporting clients through critical transitions—whether from incarceration, homelessness, or other life challenges—by addressing their mental, emotional, and social well-being through individualized assessments, counseling, treatment planning, and linkage to community resources.

As part of our innovative mobile and community-based health programs, the Clinical Case Manager meets clients where they are—both literally and emotionally—bringing high-quality behavioral health support, preventive care, and education directly into the community. By collaborating with interdisciplinary teams, healthcare providers, correctional staff, and social service agencies, this role ensures a holistic and coordinated approach to care.

Through advocacy, empowerment, and continuity of support, the Clinical Case Manager helps clients reintegrate successfully into their communities, reduce barriers to care, and improve long-term outcomes for individuals and families throughout the region.

Job Description: Community Clinical Case Manager

We are seeking a dynamic and experienced Clinical Case Manager to lead and coordinate the field operations of our Mobile Health Clinic, serving communities across Los Angeles and the Inland Empire. This position plays a pivotal role in managing a multidisciplinary team of Community Health Workers (CHWs) and Certified Peer Support Workers (CPSWs), ensuring the effective delivery of equitable, accessible, and compassionate healthcare to underserved populations.

The Clinical Case Manager will oversee behavioral health service delivery, facilitate care coordination, and promote a supportive, trauma-informed environment for both staff and patients. This role is ideal for a mission-driven professional passionate about advancing whole-person care and improving health outcomes through community-based engagement.

Key Responsibilities:

Team Leadership & Program Management

  • Lead and supervise CHWs and CPSWs, providing guidance, mentorship, and performance support to ensure high-quality service delivery.
  • Oversee daily field operations of the Mobile Health Clinic, including logistics, deployment scheduling, and supply management.
  • Collaborate closely with medical, behavioral health, and administrative leadership to ensure seamless communication, coordination, and integrated care delivery.
  • Contribute to program evaluation and quality improvement efforts, identifying opportunities to enhance service accessibility and patient outcomes.
  • Support team development by promoting a collaborative, strengths-based, and culturally responsive approach to care.

Clinical & Community Engagement

  • Conduct case management, assessments, and follow-ups for individuals with behavioral health and social service needs.
  • Coordinate care across systems—linking patients to medical, behavioral health, and social support resources within their communities.
  • Promote patient engagement and empowerment through education, advocacy, and supportive counseling.
  • Uphold confidentiality and compliance with all federal, state, and organizational regulations.

Certified Peer Support Worker (CPSW)/Community Health Worker (CHW)  Role:

 - Provide peer support services to students, drawing on personal experiences to build rapport and trust.

  • Advocate for students' health needs by assisting them in effectively communicating with healthcare providers or social service agencies.

 - Offer emotional support, mentorship, and guidance to students navigating behavioral health challenges.

  • Implement programs that promote, maintain, and improve individual and overall community health within the school setting.

 - Assist students in setting and achieving personal goals, promoting resilience and self-advocacy.

 - Facilitate peer support groups and activities to encourage positive social connections and mutual support among students.

  • Use community health needs assessments to identify areas to expand clinical services and support.

 - Collaborate with CHWs and other team members to provide comprehensive and holistic care to students.

  • Build trusting relationships with students and school staff, liaising between health and social services to facilitate access and improve service delivery quality and cultural competence.

Qualifications:

- Strong managerial skills with experience leading and managing field operations.

- Certification as a Community Health Worker (CHW) and/or Certified Peer Support Worker (CPSW).

- Experience in behavioral health, community health work, or related fields.

- Ability to build and maintain trusting relationships with students, families, and school staff.

- Excellent communication and interpersonal skills.

- Knowledge of community resources and ability to assist students in accessing services.

- Understanding of data for program evaluation and performance improvement.

- Commitment to delivering equitable healthcare and addressing social determinants of health (SDoH).

Preferred Experience:

- Lived or professional experience connecting to the student population served. This may include, but is not limited to, knowledge related to mental health conditions, substance use, homelessness, foster care, incarceration, military service, disability, domestic or intimate partner violence, or shared cultural, linguistic, or community backgrounds.

- Familiarity with the geographic communities served by the mobile school health clinic.


Key Responsibilities

Daily Operations Management:

  •  - Assist with daily site/program operations, including planning team efforts for the day or week.
  •  - Coordinate operations and logistics between teams to meet leadership goals and objectives.
  •  - Manage the team's day-to-day objectives, ensuring efficient workflow and effective service delivery.

Team Coordination and Supervision:

  •  - Supervise other CHW and CPSW team members.
  •  - Serve as a liaison between CHWs, CPSWs, clinicians, and other operational support staff.
  •  - Provide guidance and leadership to team members, conducting regular meetings to ensure alignment and effectiveness.

Resource Management:

  •  - Review supplies needed for each student outreach encounter to ensure all necessary materials are available.
  •  - Utilize technology and digital resources to monitor ongoing care activities and maintain comprehensive electronic participant files.

Program Performance Evaluation:

  •  - Evaluate program performance through key performance indicators and monitor for improvement opportunities.
  •  - Document all participant encounters, service plans, and outcomes, completing and submitting monthly reports.

Outreach and Engagement:

  •  - Conduct outreach activities within specific health programs and defined student populations.
  •  - Engage with local businesses, schools, and other relevant organizations to promote community health services.
  •  - Conduct telephonic and face-to-face outreach with students to identify social determinants of health impacting their overall wellness.

Health Coaching and Education:

  •  - Provide health coaching, patient navigation, health education, and health promotion for a diverse student panel.
  •  - Utilize coaching, motivational interviewing, trauma-informed care, and harm-reduction approaches to support students in achieving their goals.

Care Coordination:

  •  - Identify barriers to achieving targeted clinical or social outcomes and engage the care team to revise the care plan when necessary.
  •  - Assist students in accessing health-related services, overcoming barriers to medical care, and utilizing community services.
  •  - Schedule appointments with health resources and assist with completing applications for programs students may be eligible for.
  •  - Oversee the provision of Enhanced Care Management (ECM) services and implementation of care plans.

Relationship Building:

  •  - Establish positive, supportive relationships with students and provide feedback to other team members.
  •  - Build positive working relationships with students, providers, nurse case managers, agency representatives, research   staff, and office staff.
  •  - Work to reduce cultural and socio-economic barriers between students and institutions.

Advocacy and Support:

  •  - Advocate for students with healthcare professionals and connect them to other social services and supports, including transportation.
  •  - Coordinate with hospital staff on discharge plans and accompany students to office visits as needed.
  •  - Monitor treatment adherence, including medication, and provide health promotion and self-management training.

Administrative Duties:

  •  - Manage monthly and quarterly report requests from local, state, and federal entities.
  •  - Collaborate with subject matter experts (SMEs) to articulate complex facets of WEA service offerings.
  •  - Assist with proposal knowledge management and content retention for future use.
  •  - Manage non-paid staff, supervise, and develop associates based on organizational and developmental needs.

Peer Support Worker (CPSW) Role:

Peer Support and Mentorship:

  •  - Provide student peer support services, leveraging personal experiences to build rapport and trust.
  •  - Offer emotional support, mentorship, and guidance to students facing behavioral health challenges.
  •  - Facilitate peer support groups and activities encouraging positive social connections and mutual support among students.

Advocacy and Empowerment:

  •  - Assist students in setting and achieving personal goals, promoting resilience and self-advocacy.
  •  - Advocate for students' needs within the healthcare and educational systems, ensuring their voices are heard and respected.

Holistic Care Coordination:

  •  - Collaborate with CHWs and other team members to provide comprehensive and holistic care.
  •  - Support students in navigating the healthcare system and accessing necessary services and resources.

Requirements

Essential Skills and Qualifications:

As these positions represent some of the early roles to help build this program, we are specifically seeking out individuals with experience developing outreach programs and engaging communities and businesses to engage in meaningful healthcare programs


Minimum Qualifications One of the Following

  • Community Health Worker  Certificate or Certified Peer Support Worker Certificate
  • Violence Prevention Professional Certificate
  • Work Experience Pathway

Education Experience

  • Master’s degree in Social Work, Counseling, Psychology, or a related behavioral health field required.
  • Must possess either a minimum of 5 years of relevnt professional experience or lived experience
  • Ability to work independently and to collaborate with teams of individuals in diverse settings, using a solution-oriented approach.
  • Preference is given to candidates with Community Support Worker (CSW) and Certified Peer Support Worker (CPSW) credentials/certifications.
  • Current California licensure or registration as an LCSW, LMFT, LPCC, or Associate-level clinician (ASW, AMFT, APCC) required.
  • Minimum 3–5 years of experience providing case management or behavioral health services, preferably within community-based, mobile, or integrated healthcare settings.
  • Demonstrated history of strong interpersonal skills and ability to understand and follow written/verbal instructions.
  • Demonstrated knowledge of local and regional community resources.
  • Demonstrated knowledge of public health programs.
  • Skilled in utilizing appropriate industry standard assessment techniques.
  • Demonstrated ability to provide appropriate guidance and positive customer service using a patient-centered approach.
  • Must possess a comprehensive knowledge of the local community based on personal lived experience or the ability to articulate the lived experience and perspective
  • Preference given to bilingual Spanish speakers. 
    • Preference to cultural competence with LatinX communities
