We achieve this by offering multiple levels of care along with medical services, evidence-based professional counseling, case management, housing, employment assistance, and more throughout the Phoenix metro area.
Duties/Responsibilities
- Provide individual, Family and group substance use counseling with a trauma informed care approach.
- Coordination of care with stakeholders, funding sources and natural supports.
- Provide culturally appropriate modalities in support of client’s cultural preferences.
- Develop, implement, and review clinical documentation of all services provided through client records in the Electronic Medical Record (i.e., assessments, ASAM Continuum, individualized service plans, screening tools, safety plans, progress notes, discharge summary, etc.) for assigned caseload.
- Oversees the accuracy and completion of each assigned client’s record in Electronic Medical Record.
- Collaborates with each assigned client on individualized service plan goals and Adult Recovery Team meetings throughout client’s episode of care.
- Assess, monitor and refer to appropriate level of care for each assigned client.
- Actively participate in interdisciplinary treatment team meetings/staffings.
- Complete accurate clinical documentation same day as date of service.
- Stay current with updated research and best practices.
- Secure client information per HIPAA standards.
- Attends and/or provides in-service training as needed.
- Actively work with Interns as needed under the oversight of Clinical Leadership Team member.
- Promotes inclusion through Culturally and Linguistically Appropriate Service Standards (CLAS).
- If licensed with AZBBHE, maintain licensure in good standing with AZBBHE and complete all continuing education requirements.
- Perform other duties as assigned.

Requirements
Applicants must possess at least one of the following licenses in Arizona: LAC, LPC, LASAC, LISAC, LMSC, LCSW, or Psychologist.
- Graduation from an accredited college or university with a minimum of a Master's Degree in Psychology, Social Work, Counseling, or health-related field
- Master’s or Bachelor’s Degree and Licensed with the Arizona Board of Behavioral Health Examiners (LAC, LPC, LMFT, LMSW, LCSW, LAAC, LIAC) preferred
- Two to three years of experience working in behavioral health or substance abuse field preferred.
- Must be proficient in computer applications and electronic medical record systems.
- Proficient with Microsoft Office and related software.
- Fingerprint Clearance Card upon hire
- Current First Aid/CPR certification within 90 days of hire.
- Negative TB Test result within the last 12 months.
- Ability to communicate effectively with clients, their loved ones and professionals in both written and oral format.
- Ability to make calm sound decisions in emergency situations.
- Ability to read, analyze, and interpret human service periodicals, professional journals, technical procedures, or governmental regulations.
- Strong assessment skills, time management skills, and clinical skills.
- Ability to work independently and as part of a team
- Working knowledge of ASAM.
- Ability to write reports, business correspondence, and analyze policies and procedures.
- Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
- Experience in working with diverse population and socioeconomic backgrounds.
- Experience with working with Native American population preferred.
- Working knowledge of twelve step programs of recovery and other models of self-help options in the community.
