This is a full time, salaried position. Work hours: Monday through Friday, 8:30 am to 5 pm.

As our member of our team, you will have the opportunity to help set the standard, providing comfort and support to our residents and their families. We are seeking a talented social worker who values older adults, to advocate and help guide our team to provide outstanding customer service.
As our Social Services Coordinator your duties would include:
- Conduct comprehensive assessments of residents and their families to determine their needs and develop individualized care plans.
- Provide counseling and support to residents and their families to help manage physical, emotional, and social challenges.
- Facilitate communication and collaboration between residents, their families, healthcare providers and Misericordia.
- Advocate for resident rights and interests and ensure that their needs are met in a timely and appropriate manner.
- Develop and maintain effective relationships with community-based organizations, government agencies and other partners to enhance the delivery of service to our residents.
- Provide education and resources to residents and their families on various aspects of long-term care.
- Assist our Rehabilitation clients and their families with their return to home, or other discharge plans.
You will use your assessment skills, make decisions, and help the team to serve our residents. We value the experienced, who have a purpose-driven and collaborative approach to care.
Requirements:
- Genuine concern for the needs of older adults.
- Strong customer service skills with both internal and external customers.
- Bachelor of Social Work is required.
- Experience in a long-term care setting is preferred.
- Knowledge of state and federal regulations related to long-term & rehabilitation care services.
- Ability to work independently and as part of a team.
- Experience with electronic health records (EHR).
This is a full time, salaried position. 8:30 am to 5 pm. Monday to Friday. No Weekends.
