We are looking for an Assistant Director of Health Services (ADHS) to join our team! This position is responsible for ensuring that the safety and health needs of the clients are met while providing staff leadership duties. This nursing leadership schedule includes;
- 80 hours every two weeks
- Full time
- Every third weekend rotation
Responsibilities:
- Maintains current assessment and documentation of client’s health status.
- Performs initial and on-going assessments per the applicable Minnesota Assisted Living regulations and Nurse Practice Act. Ensures RN Assessment is completed upon admission, within 14-days of admission, and at least every 90 days and when/if there is a change in health condition.
- Works with client and/or responsible party through collaboration, education and timely communication to ensure appropriate service and placement.
- Assures that clients have up-to-date service agreements. Implements service agreements with new tenant/residents. Reviews and updates service agreements at least once per year and more frequently when there is a change in service needs.
- Develops, maintains, and implements individual emergency plans based on health status and Advance Care Planning documents, including POLST.
- Maintains clinical charts and progress notes using computerized documentation system.
- Communicates changes in client’s conditions to medical and other relevant individuals.
- Coordinates client health services.
- Facilitates coordination of services with internal and external providers.
- Assures that all physician orders are signed and implemented per the applicable Minnesota Assisted Living regulations.
- Works with client and/or representative(s) to assure delivery of appropriate services and funding.
- Participates in Care Conferences as needed or requested by the DHS or Executive Director
Required:
- Vocational/Technical Training or Associate Degree
- LPN Licensure/Registered with the MN Board of Nursing
Current Basic Life Support (BLS) for Health Care Provider from American Heart Association (AHA) or American Red Cross
BLS is required upon hire and/or new hire must show proof that they are currently enrolled in a BLS class to renew or obtain their certification. BLS certification must be obtained within three-weeks of hire date and before nurse can perform independent practice
- One to three years of previous experience
Preferred:
- Bachelor's Degree: Nursing
- Three to five years of previous experience
- Prior supervisory experience
- Prior experience in home care or assisted living environment
$38.80- $56.04 Hourly
