POSITION SUMMARY:
Responsible for conducting community outreach, recruiting participants for study, providing referrals to needed services, data collection and organizing workshop activities within the Williamsburg/Brooklyn and Lower Manhattan community.
DUTIES AND RESPONSIBILITIES:
1. Work directly with the program manager & VP of ID/LGBTQ Programs and Services.
2. Conducts street outreach to recruit participants for the Mobile
3. Review data collection with program manager
4. Maintain all files and records, including the outreach activity.
- Meets with program manager/ VP of ID/LGBTQ Programs and Services on a bi- weekly basis to discuss productivity and any issues that arise during stage-based outreach.
- Maintain program database.
- Organize the outreach sites
- Refers clients for other services in CHN, including ADAP services, Medical appointments and other services pertaining to clients needs.
- Documents clients that were reached at each activity.
- Facilitate opportunities to get involved in community events.
- Distributes education materials to clients.
- Schedules and conducts focus groups and outreach activities.
- Participate in trainings and in-service workshops
14. Facilitates related services for health center patients and CHN staff as appropriate, with respect to their confidentiality and privacy.
- Completes monthly statistical reporting requirements
- Assists in the daily functions on the Mobile as needed.
- Performs other related duties, as assigned
Certificate/License required:
· High School diploma or GED
