The Lead Medical Social Worker provides high-quality geriatric social work support to a panel of assigned participants. They also serve as a peer mentor, subject matter expert in social work and provide the social work department with clinical and administrative support.SPECIFIC RESPONSIBILITIES:
- Completes all of the responsibilities of a Medical Social Worker for an assigned panel of participants.
- Provides clinical support, consultation, and guidance to social work team members, especially related to complex cases and/or situations.
- Assists with onboarding new Medical Social Workers, Case Managers and/or Social Work Assistants. Provides shadowing and mentoring on departmental workflows and standards.
- Serves as a departmental representative in internal and external workgroups to ensure that social work feedback is incorporated into cross-functional workflows and projects.
- Works closely with assigned supervisor and program leadership team to develop, implement, evaluate, and monitor social work policies and procedures, workflows, best practices, documentation standards, and regulatory requirements.
REQUIREMENTS:
- Must hold an MSW from an accredited school of Social Work.
- At least three years of professional experience working with the frail elder population.
- At least one year of experience in the PACE model of care is strongly preferred.
BACKGROUND AND EXPERIENCE:
- Experience with and understanding of the medical and psychosocial problems of functionally impaired adults and the frail elderly.
- Exceptional communication and presentation skills relating to the frail elderly, functionally impaired adults, their support systems, and teams of health professionals.
- Demonstrates case management skills and experience in the community health care delivery system.
- Detail oriented with good problem-solving skills and the ability to prioritize multiple tasks.
- Bilingual in English and either Cantonese or Spanish preferred.
COMPENSATION
Range: $95,170 - $113,742/annual
This rate is not necessarily reflective of actual compensation that may be earned, nor a promise of any specific pay for any specific employee, which is always dependent on actual experience, client needs, education and other factors.
