The Health Outcomes Specialist I reviews new patient charts in advance of appointments to ensure records are accurate, comprehensive, and up to date. This position supports both clinical readiness and proactive patient outreach efforts.
Responsibilities of this position include:
- Collects and accurately enters data from multiple sources into the electronic health record (EHR) to ensure comprehensive patient records. This may include paper records, external data sources such as other medical record sources, health information exchanges, immunization registries, and claim records. Submits a request for records as required, including utilizing the Release of Information (ROI) sheet.
- Accurately records patients’ histories, procedures, allergies, problems, and medications directly into the EHR.
- Utilizes registry data, medical record recommendations, as well as gap lists to determine and outreach for preventative care needs of the patients.
- Assists in outreach efforts to encourage and recommend patients to receive preventative and maintenance services for wellness and chronic conditions.
- Provides effective and timely communication with providers, staff, and care teams regarding quality gaps for their patients.
- Recommends or proposes orders within the electronic health record such as labs, appointments, services that have been identified as gaps in care.
- Participates and collaborates in meetings and process improvement efforts related to patient care gaps.
Requirements for this position include:
- High school diploma or GED required
- Associates degree preferred
- Healthcare background; experience working with electronic medical records preferred
- Knowledge of medical terminology & patient charting. Ability to effectively communicate, verbally, orally and in writing; must interact well with coworkers, patients, physicians & the general public. Proficient computer skills.
- BLS certification required
- CNA license preferred
