GENERAL SUMMARY:
Under the administrative supervision of the Clinical Coordinator of the Community Participation Supports Program, the position is responsible for the educational and vocational development of assigned individuals enrolled in the Community Participation Supports Program of the William Warren Scranton Vocational Center.
EDUCATION:
Bachelor degree in a professional category specified in the Department of Public Welfare, Vocational Facilities Regulations (2390.33) required.
WORK EXPERIENCE:
Minimum of three (3) years experience working directly with development disabled individuals required.
Maintains case records, prepares required correspondence, completes written reports, monitors utilization of billing units and updates statistics as necessary for the efficient operation of the program in compliance with 2390 regulations and Quality Assurance & Improvement requirements. EIM point person for HCSIS.
Actively participates, coordinates, integrates, and monitors active treatment services provided to the individual in accordance with the Base Service Unit MH/ ID Individual Support Plan
Conducts personal, vocational, and social adjustment counseling to individuals in an individual or group setting.
Provides behavioral management services and support and/or applies Crisis Intervention Techniques as needed. Assists Direct Support Professionals in the implementation of these plans.
Provides direction and assistance to program staff in implementation of individual’s established outcomes. Develops and implements specific techniques to allow for maximum individual social and vocational development in accordance with the Individual Support Plan.
With the Base Service Unit coordinates, schedules, and participates in meetings of the individuals’ interdisciplinary team to review the individuals’ program plan in accordance with prescribed statutes and regulations. Participates in inter-agency team meetings at various sites as required
Develops, in conjunction with Base Service Unit Behavior Specialist, all plans of support dealing with aggressive and non-cooperative individuals and implements such plans as expected
Assists Direct Support Professionals in meeting production quotas while maintaining quality requirements. Provides support services as needed for Direct Support Professionals during lunch/break times during the occurrence of absences to maintain compliance with the Department of Welfare 2390 regulations.
With the Clinical Coordinator, participates in the interview process/screening of individuals for admission to the program. Interprets program procedures
and practices to individuals and their treatment teams/families/guardians.
Provides in-service training as needed providing internal and external educational experience for staff relating to their responsibilities, daily program operation, and policies and procedures.
Completes discharge process, compiles necessary documentation in accordance with existing regulations. Provides follow-up services for all assigned individuals who progress to higher level rehabilitation programs or terminate services.
With the Clinical Coordinator, performs a periodic review of the total program and evaluates its effectiveness for the individuals involved to determine long-term program goals (Quality Management Goals).
Works with administration and staff to plan and develop a comprehensive program to promote the individuals’ social and functional skills to facilitate maximum functioning and independence in the program for a successful placement into the competitive job market. Will directly support the individual within the community as needed as part of the Community Supports Participation Program.
