MDS Coordinator/Nurse Manager
Full-time.
Starting Wage: $35-48.22/hr. depending on experience.

About the Role:
As a MDS Coordinator/Nurse Manager at Columbine Commons, you will:
Oversee the quality of care on assigned units.
Serve as a coach and resource for nurses and CNAs.
Collaborate with Charge Nurses regarding acute issues.
Monitor compliance of documentation for events and appropriateness of documentation.
Ensure Skilled Stay documentation covers all necessary areas.
Follow-up on falls and skin issues.
Collaborate on the plan of care for residents exhibiting expressions of need.
Ensure RIS reflects the needs of the resident and staff is referring to it.
Review MAR/TAR for appropriate orders, streamline med pass times if needed, discontinue treatments no longer current, etc.
Educate, train, and orient staff on the Resident Assessment Instrument (MDS and RAPs).
Ensurs timely and accurate completion and submission of the RAI
Obtain the Final Validation Reports for the MDS submission and ensures correction of all errors.
What We're Looking For:
Experience and Education:
Nursing degree from accredited college or university required.
Clear unencumbered license from the State of Colorado required.
1+ years of relevant experience and/or training.
Knowledge of the aging process, dementia, and disabilities preferred.
Experience with geriatric population in long term care facility, assisted living facility, or other related health care facility greatly preferred.
Skills:
Clinical oversight and assessment.
Staff leadership and coordination.
Regulatory compliance and quality improvement.
