Key Responsibilities

  • Manage day-to-day operations of all value-based care programs, including CMS CKCC program, Chronic Care Management and Interwell Acumen Optimal Performance Program.
  • Implement workflows and processes that support participation and success in all value-based care programs (e.g., shared savings, capitation and Chronic Care Management initiatives)
  • Translate payer contract requirements into clear operational procedures and ensure accurate execution at the practice level
  • Establish, monitor, and report key performance indicators (KPIs) tied to quality, utilization, and financial outcomes.
  • Partner with IT, analytics, and Acumen teams to develop and maintain dashboards and reporting tools that support data-driven decision-making.
  • Collaborate with providers, care teams, and leadership to enhance understanding, engagement, and accountability in value-based initiatives.
  • Oversee risk adjustment, documentation accuracy, and coding workflows to optimize performance under value-based contracts.
  • Lead, coach, and develop a high-performing team that fosters collaboration, accountability, and continuous improvement 


Requirements

  

  • Bachelor’s degree in healthcare administration, business, nursing, or a related field.
  • Minimum of 3-5 years of experience in healthcare operations, value-based care, or population health management with 2-3 years in a supervisory role.
  • Proven experience with data analytics and analysis tools, healthcare IT systems, and performance management tools. 
  • Proficiency in Microsoft (Word, PowerPoint, Excel, & Teams), EHR systems, and Care Coordination tools.
  • Excellent communication, problem-solving, and interpersonal skills.
  • Regular, reliable, and punctual attendance is an essential function of this position. The ability to consistently adhere to      scheduled work hours and fulfill job responsibilities in a timely manner is critical to the success of the role and the overall operations of the team.
