Description
The Outreach & Enrollment Specialist bridges school-based and community programs with CNYHHN Health Home (HH) Care Management. The role’s main purpose is to identify, engage, and enroll eligible individuals in HH services while maintaining a limited care management caseload to ensure service continuity.
Requirements
Activities include but are not limited to the following:
HH Care Management Liaison for Connect ED and Its Programs
- Develop a comprehensive understanding of the Connect ED Community Schools Initiative, including RIYS, Resiliency, and The Go Program, to strengthen collaboration between schools, the community, and HH Care Management services.
- Collaborate regularly with Connect ED staff, including Site Coordinators and Project Managers, to identify students and families who may benefit from HH Care Management.
- Actively participate in School-Based LINK meetings and other applicable interdisciplinary team meetings related to assigned cases.
Outreach & Engagement
- Conduct proactive outreach and engagement with parents/caregivers who express interest in HH Care Management for identified students and eligible household members.
- Review assessments, attendance data, behavioral indicators, and other program data to identify families at risk and determine the need for outreach or re-engagement.
- Build trusting relationships with families to support informed decision-making and connection to services.
Enrollment & Intake
- Complete HH Care Management intake and enrollment processes for all populations, including Health Home Serving Children (HHSC) and Health Home Serving Adults (HHSA).
- Gather required documentation and ensure compliance with enrollment standards, including consent forms, assessments (e.g., CANS-NY, HARP), and verification of eligibility.
Limited Care Management Caseload
- Maintain a small, active caseload to provide continuity of care and direct support to enrolled members.
- Conduct care plan reviews, make referrals, and coordinate with interdisciplinary teams to ensure timely access to services.
- Document all activities accurately in accordance with CNYHHN and state Health Home standards.
Data Review & Performance Tracking
- Review monthly data on HH Care Management referrals generated through Connect ED programs, including:
- Number of HHSA and HHSC referrals by site/program.
- Number of successful enrollments.
- Instances of disengagement or denial of services following outreach.
- Identify trends and make recommendations to improve outreach efficiency and referral conversion rates.
Committee Participation & Continuous Improvement
- Actively participate in the HH/Connect ED Committee, providing updates on referrals, case status, and cross-program workflows.
- Contributes to process improvement discussions related to referral tracking, documentation, and communication between Connect ED and HH teams.

QUALIFICATIONS / EDUCATION / EXPERIENCE REQUIREMENTS
- A Bachelor’s degree in one of the following fields listed: a major or concentration in social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreational therapy, counseling, community mental health, child and family studies, sociology, speech and hearing or other related human services field; and two years of experience In providing direct services to people with Serious Mental Illness, Developmental Disabilities, or Substance Use Disorder; OR in linking individuals with Serious Mental Illness, Developmental Disabilities, or Substance Use Disorders to a broad range of services essential to successful living in a community setting (i.e. medical, psychiatric, social, educational, legal, housing and financial services); OR
- A NYS teacher’s certificate for which a bachelor’s degree is required; OR NYS licensure and registration as a Registered Nurse and a bachelor’s degree; OR A Bachelor’s level education or higher in any field with five years of experience working directly with persons with behavioral health diagnoses; OR A Credentialed Alcoholism and Substance Abuse Counselor (CASAC); OR A Master’s Degree in one of the qualifying education fields may be substituted for one year of experience.
- Basic Computer Skills (Windows, Outlook, Word, Excel)
- Travel is required. Must have a vail NYS Driver’s License.

- This position has the potential for regular and substantial contact with health home enrollees under age 21 and must satisfactorily pass a Criminal History Record Check (including fingerprinting), State Registered Clearance, Mandated Reporter Training, and Staff Exclusion List.
WORK ENVIRONMENT / HAZARDS
- Job related tasks do not involve exposure or potential exposure to blood, body fluids, or tissue and Category I tasks are not a condition of employment. May have exposure to unpredictable individuals and situations when working at CNYHHN sites, its affiliates or the community.

- Certain deadlines and unanticipated developments may require work during evenings, weekends.
- Ability to quickly address any emergent issues without losing focus on task at hand.
- The employee must have full sight and hearing with fluency in the English language. While performing the duties of this job, the employee is regularly required to talk or hear.
- The employee frequently is required to stand, walk and sit. Must be able to sit for long periods of time.
- Constantly operates a computer and other office productivity machinery, such as a copy machine, scanner, computer printer, etc.
