What you will do:
Review daily hospital visit reports including HSX mailbox/MCO reports. Identify hospital discharges and prepare site-based list for follow up by site-based care coordinators/CHWs. Complete documentation in EHR and/or on reports to monitor progress. Manage high risk lists from special program such as Health Partners PCMH, Keystone First PCMH, DVCH PCMH lists. Generates and reviews list to identify patient needs. Coordinates phone calls and schedules patients with PCP/Care coordinator and CHW. Reviews Care gaps for special program patients and supports PI projects/ PCMH initiatives. Completes Transition of care (TOC) follow up. Identifies patients who did not show to appointments, contacts patients, analyzes reason for missed visits and supports patients with follow up. Support medical providers with DME supplies or letters as appropriate. Sets up ASL translator services. Supports medical providers with home care/wound care referrals. Contacts home care agency and obtains report to ensure continuity of care with medical provider. Attends home visits with CHW as necessary. Completes other dues as requested by Director of Care Coordination

Requirements

  • Graduation from accredited LPN required. 
  • Current PA license as an LPN. 
  • CPR certification required. 
  • Preferred experience in primary care, urgent care, emergency care, home health and/or ambulatory care. 
  • Demonstrated use of technology as a platform for patient care and connected clinical devices. 
  • Must possess strong interpersonal skills and the ability to work effectively with a diversity of internal and/or external contacts.
  • Must be able to present a positive and enthusiastic demeanor while directly working with patients. 
  • Experience with case management or utilization management preferred. 
  • Bilingual skills (English/Spanish) strongly preferred.
