PURPOSE OF THIS POSITION
The purpose of this position is to supervise the day to day clinical and administrative operations of the Hospice Social Workers, Spiritual Care, Volunteer program, and Bereavement program, in coordination with the Hospice Medical Director, Hospice leadership, and IDG. This position reports to the Hospice Manager.
JOB DUTIES/RESPONSIBILITIES
Duty 1: Demonstrates the ability to support the mission of the Hospice team in providing consistent, high quality patient care to those being referred to or receiving services, through maintaining own schedule of patients and completion of Hospice social work evaluations and visits, following all regulatory documentation and billing requirements for services.
Duty 2: Demonstrates the knowledge and skill to mentor the Hospice social services teams in providing consistent, high-quality patient care to those being referred to or receiving Hospice services.
Duty 3: Collaborates with Hospice management, intake staff, and scheduling assistant in coordination of schedules of the Hospice Social Workers and Spiritual Care team members in relation to the Plan of Treatment. Assists with assigning primary patients and/or visits per Bridge Home Health and Hospice Guidelines.
Duty 4: Collaborates with Hospice management and Medical Director to ensure routine IDG meetings and notes are completed following requirements set forth by regulatory and accrediting bodies.
Duty 5: Supervises the clinicians in performance of their daily duties and assists clinicians with assessment/intervention/evaluation skills; reports concerns to Hospice management.
Duty 6: Collaborates with the Bereavement and Volunteer Coordinators, Hospice management team, and Community Nurse Liaisons to ensure events and activities are planned and executed.
Duty 7: In collaboration with Hospice management and clinical education, plans and participates in the orientation of new associates and in the on-going education of current associates, and facilitates interdisciplinary and team educations within the Hospice team.
Duty 8: Ensures personal documentation, as well as all other social services documentation is consistent with or exceeds Bridge Home Health and Hospice guidelines.
Duty 9: Coordinates with Hospice management for oversight of Hospice Social Workers, Spiritual Care team members, Volunteer Coordinator, and Bereavement Coordinator, including staff performance evaluations.
Duty 10: Displays Service Excellence as evidenced by practicing the mission, vision, and values of the organization to promote patient satisfaction.
Duty 11: Complies with organizational policies, procedures, and practices to ensure quality job performance and compliance with applicable regulatory and accreditation standards.
Duty 12: Completes other duties as assigned.
REQUIRRED QUALIFICATIONS
Master’s degree in social work
Current state licensure as a Licensed Social Worker (LSW)
Ability to perform duties of a direct patient care Social Worker.
A valid driver's license is required (if you do not have a valid Ohio driver’s license you must obtain one within 30 days of your residency in the state). You must also meet BVHS's company fleet policy and insurance company requirements, and any other requirements that may be required to operate a vehicle.
Positive service-oriented interpersonal and communication skills required.
Individual must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served on his/her assigned unit/department. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient status. Must be able to interpret the appropriate information needed to identify each patient’s requirements relative to their age-specific needs and to provide the care needed as described in the area’s policies and procedures
PREFERRED QUALIFICATIONS
2 years of Hospice experience in an inpatient and/or outpatient setting with a working knowledge of Hospice Care and Palliative Care
Prior supervisory experience
PHYSICAL DEMANDS
This position requires a full range of body motions with intermittent walking, lifting, bending, squatting, kneeling, twisting, and standing. The associate will be required to walk for up to one hour a day, sit for four to six hours a day and stand for one hour a day. The individual must be able to lift twenty to fifty pounds and reach work above the shoulders. The individual must have good eye-hand coordination and fine finger dexterity for simple grasping tasks. The individual must have excellent verbal communication skills to perform daily tasks. The associate must have corrected vision and hearing in the normal range. The individual must be able to operate a motor vehicle for business travel and community involvement.
