Hematology Oncology Associates was established in Palm Beach County over 35 years ago, and has grown to include 10 Board Certified Hematologist and Oncologists and 6 Nurse Practitioners in 3 convenient locations within our community. Our goals are to provide state of the art cancer treatments with a high level of compassionate care in a warm inviting environment. We also specialize in treatment of coagulation blood disorders, leukemias and other hematologic benign conditions. Job Description:
- Evaluate incoming patient referral documents for completeness and accuracy.
- Communicate with referring and attending physicians to clarify medical information or obtain additional details.
- Work closely with the Medical Records department to obtain any missing documentation based on the patient’s diagnosis and scheduling requirements.
- Contact patients to schedule new appointments, prioritizing based on medical urgency and clinical guidelines.
- Review and evaluate incoming patient medical records and referral documentation to determine the complexity and urgency of diagnoses.
- Collaborate with physicians and clinical teams to assess the appropriate level of care and identify the best-suited provider and appointment type.
- Schedule new patient appointments with attention to diagnosis severity, physician availability, and continuity of care.
- Communicate appointment details, preparation instructions, and care expectations to patients clearly and compassionately.
- Coordinate with insurance verification and authorization teams to ensure pre-appointment requirements are met.
- Document all patient communications and scheduling activities in the electronic medical record (EMR) system.
- Triage calls and inquiries related to new patient scheduling, routing complex clinical questions to appropriate staff.
- Track and follow up on incomplete referrals or missing documentation to avoid delays in patient care.
- Support quality and performance improvement initiatives related to patient access, care coordination, and scheduling accuracy.
