Each year, more than 400 children, adults and their families receive services from the organization.%3Co:p%3E%3C/o:p%3E%3C/p%3E%3Cp style=%22line-height:115%;margin-bottom:7pt;margin-right:0in;margin-top:0in;%22%3E%3Cbr%3E%26nbsp;%3C/p%3E%3Cp style=%22line-height:115%;margin-bottom:7pt;margin-right:0in;margin-top:0in;%22%3E%3Cbr%3E%26nbsp;%3C/p%3E%3Cp style=%22line-height:115%;margin-bottom:7pt;margin-right:0in;margin-top:0in;%22%3EJob Summary:%3Cbr%3E• Provides informal training and direct care to clients based on person-centered%26nbsp; %26nbsp; plans, individual needs/requests and daily household operations.%3Cbr%3E%3Cbr%3EResponsibilities:%3Cbr%3E• Supervises, trains, and/or assists client in accordance with the person-centered plan, including personal care, recreation, and reinforcement of%26nbsp;individual habilitation objectives.%3Cbr%3E• Assures client%27s personal property and adaptive equipment is clean and in%26nbsp;good order including wheel chair, clothing and living space. Plans, schedules and transports clients to work, appointments and community activities.%3Cbr%3E• Documents routine medications and treatments as prescribed by physicians and/or health care professionals.%3Cbr%3E• Assists individuals with meal preparation and encourages good nutritionalhabits and adherence to prescribed diets.%3Cbr%3E• Effective oral, written and electronic communication skills.%3Cbr%3E• Assists in all duties essential to the operational needs of the site.%3Cbr%3E• Assists persons served to meet their physical, emotional, spiritual,%26nbsp;developmental, health an safety needs.%3Cbr%3E• Adapts positively to a fast paced changing environment.%3Cbr%3E• Maintain required licenses, certifications and trainings.%3Cbr%3E• Creates and completes all related paperwork for mandatory compliance with DDS and other agencies.%3Cbr%3E%26nbsp;%3C/p%3E

 

Licensure / Certification / Registration:

• Required Credential(s):
• Licensed Drivers License credentialed from the Connecticut Department of

  Motor Vehicles obtained prior to hire date or job transfer date.

Education:
• High school diploma or equivalent. BA/BS preferred.

Work Experience:
• Three years experience working with individuals with disabilities preferred.
