Case Manager – Health First Colorado:

 The Health First Colorado (HFC) Case Manager is responsible for the delivery of patient care services through coordination, implementation, and supervision of current or potential HCF patients.  This position will focus on outreach to the HCF population in efforts to provide proactive and responsive interventions to assist members in accessing evidence - based preventative care services.

 

Position Summary:

 With a Quality, Customer First, and Compassionate approach, The Case Manager – Health First Colorado will:

  • Identifies HFC clients that will benefit from outreach and care coordination.  Works with agencies and service providers to facilitate care coordination and information sharing.  
  • Provides care management to HFC patients. 
  • Screens HCF patient for healthcare needs with additional focus on key performance indicators (KPI)
  • Track outcomes and report data to support performance improvement and KPI goals (e.g., preventive care, ED reduction).
  • Documents patients’ case management plans and ongoing activities.         Charts data in Electronic Health Record (EHR).   
  • Team Based Care – Clinical Care Collaboration – Works in collaboration and continuous partnership with providers, clinical staff, and community resources in a team approach to promote access to appropriate care in the goal of enhancing patient health.  
  • Identifies patient needs, current services, and available resources and connects the patient to services and resources to meet established goals.
  • Identifies and addresses patient risk factors and/or barriers to care. 
  • Communicates the care preferences of patient, serving as their advocate, and verifying that interventions meet the patients’ needs and goals of treatment.  
  • Educates the patient/family/caregiver about the case management process and evaluates their understanding of the process.        
  • Monitors patients according to the guidelines set by the CDC, CCGC, State and Sunrise Community Health.
  • Assists with scheduling appointments for follow-up care, referrals, and medication refills.    
  • Attends assigned trainings.  
  • Provides health coaching and patient education for HCF patients. 
  • Assists patients with transportation needs.
  • Other duties as assigned.  

 

Minimum Qualifications:

  • High School Diploma, and/or GED required.        
  • Medical Assistant certification or equivalent combination of education and experience preferred; or 1–2 years of related experience and/or training.
  • Bilingual English/Spanish preferred

Current immunizations are required to work at Sunrise Community Health and may vary dependent upon the position Influenza (Flu) Vaccines are required for ALL staff. COVID vaccine is highly encouraged. N95 Testing is required for Clinical positions and facial hair must not interfere with the seal or valve function of the respirator.
