RESPONSIBILITIES:

  • Evaluates the drug regimen reviews of patients/clients in the facility, assuring appropriate drug therapy.
  • Monitors the compliance of the facility with federal and state guidelines and notes any irregularities.
  • Serves as an information source regarding medication and federal/stale pharmacy regulations.
  • Acts as a liaison between the pharmacy and the facility when problems need to be resolved.
  • Educates nurses about problem areas within the facility and also in areas of staff interest.
  • Attends necessary meetings of the facility, pharmacy, infection control, quality assurance, and interdisciplinary team conference.
  • Review UDA for analysis of usage reports and report to QA.
  • Collaborate with the QA committee to develop strategies to maximize appropriate antibiotic utilization and to minimize inappropriate antibiotic utilization to improve upon positive patient outcomes, minimize discharge to hospital and to minimize antibiotic resistance.
  • Receives physician orders and evaluates for appropriate action, dosage, potential drug interactions, administration and stability.
  • Assists in training and supervising pharmacy technicians, interns and externs.
  • Produces written reports on findings and recommendations for each monthly visit to facilities and provides quarterly reports to the administrator, DON, and where appropriate, the medical director and resident's physician. These reports include findings related to medication therapy and the monitoring of medication therapy.
  • Reconciles controlled drugs and audits patient charts.
  • Travels frequently, often on short notice.

 

REQUIREMENTS:

  • B.S. in Pharmacy
  • One or more year's pharmacy experience preferable in long-term care; detailed knowledge of dispensing systems and general knowledge of medical consulting. 
  • Licensure in assigned state. Excellent writing and oral communication skills.
