Description
Job Summary:
- Evaluate patients; develop a plan of care; provide treatment to patients; and perform OASIS assessments, case conferences, and summaries of care at 30 days, at 60 days and at discharge.
- Act in the role of lead supervisor for LPTA and OT staff.
- Assist in maintaining staff education with regard to functional assessments, safety and fall prevention.
- Complete documentation according to agency policy.
- Work with OT and scheduler to schedule 11-13 visit assessments, 17-19 visit reassessments and 30 reassessments.
- Demonstrate and encourage team behavior and exceptional patient/guest experiences.
- Uphold and promote patient safety and quality.
Qualifications
Education:
Minimum of bachelor's degree in physical therapy. May also have a master's or PhD in physical therapy. Licensed in the state of Alabama.
Experience:
Minimum 2 years of experience as a physical therapist. Preferred experience in home care. Supervisory experience preferred.
Additional Skills/Abilities:
Should possess good leadership skills, great communication skills, critical thinking skills, flexibility, and the ability to multitask and prioritize. Must be organized and driven to complete work independently.
