Palliative Medical Assistant
Location: Tacoma, WA offices
Schedule: Full-time, Monday to Friday, 8:00 am to 5:00 pm
Puget Sound Home Health & Hospice is seeking a detail-oriented and collaborative Palliative Medical Assistant to join our team.
This pivotal role for community-based palliative care/chronic care management (CCM) provides both clinical and administrative support to a care team that serves patients with serious, long-term illnesses. This role involves working closely with patients and families in their homes and communities to help manage symptoms, coordinate care, and improve overall quality of life.

Qualifications
- Graduate of an accredited Medical Assistant program; current MA certification required (AAMA, AMT, NHA).
- Current Basic Life Support (BLS) certification.
- Experience in palliative, hospice, or chronic care preferred.
- Strong communication, organization, and problem-solving skills; empathetic and calm under pressure.
- Proficiency with Electronic Health Records (EHR) and virtual health technologies
- **Experience with Home Care Home Base is strongly preferred
Key Responsibilities:
Patient Care & Support
- Take vitals, document patient concerns, and report changes to providers.
- Coordinate care plans, schedule appointments, arrange transportation, and manage referrals.
- Reinforce provider instructions on symptom management, medications, and follow-up care.
- Provide emotional support to patients and families, assisting with ADLs and offering information as directed.
Administrative Support
- Maintain accurate and timely electronic health records (EHR) and care documentation.
- Conduct outreach to patients with chronic conditions via phone, email, or virtual visits.
- Serve as a communication hub between patients, families, providers, and community partners.
- Support front desk operations, scheduling, and supply management; ensure exam rooms are prepared and stocked.
