- Finding resources to assist the patient with: transportation, obtaining medications, financial barriers, social security or employee-based disability claims, or other barriers to safe and successful treatment.
- Collaborates with behavioral health team to meet the needs of high risk populations.
- Helps to facilitate care transitions and other specialties when needed.
- Develops and maintains community relationships to support client referrals.
- Consults and collaborates with other professionals involved in providing services to patients.
- Attends ROSC-related meetings in or out of the office including with psychiatric and medical prescribers as necessary.
- Documents and provides data entry for grant reporting and tracking for services delivered through grant funded programs
- Keeps the director/manager informed of problematic cases, especially those involving legal or risk management issues.

Salary Range:The compensation range for this position is $18.39per hour - $29.79per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked and compensation model.
