What You'll Do: 

• Monitor client conditions, perform focused assessments, and report findings and changes to the RN in a timely manner
• Ensure unlicensed staff provide high-quality, timely care and address performance or training needs with the RN
• Implement and document physician orders, assist with care plan updates, and ensure compliance with agency policies and state regulations
• Administer medications accurately, maintain proper documentation, and respond promptly to emergencies or incidents
• Support client admissions, transfers, and discharges while maintaining open communication with clients, families, and interdisciplinary teams

What You'll Need: 

• Active MN LPN License
• Minimum of 1 year of experience in home health, med-surg or acute care preferred
• Prior Supervisory experience preferred
