JOB SUMMARY: 


The RN is responsible to manage quality and process improvement activities in coordination with practice services, assist in the development of the annual Quality Improvement work plan. Evaluate and analyze the effectiveness of practice services and quality improvement activities and initiate and/or provide oversight of new and ongoing programs/projects related to clinical and service process improvement ensuring compliance with internal as well as external regulatory reporting agencies and accrediting bodies.


Minimum Job Qualifications:


Education:

  • Bachelor’s Degree required.
  • Specific coursework in quality improvement, continuous improvement theory, operational analysis, or other quantitative evaluation methodology preferred.


Experience:

  • Minimum of three years of experience in a medical practice setting preferred.
  • Minimum two years of experience in project management, clinical data collection and analysis, quality assessment and improvement processes preferred.


 License/Certifications:

  • Current Registered Nurse (RN) licensure in the state of NC.
    Current BLS Certification.
  • Certified Professional in Healthcare Quality CPHQ preferred.


ESSENTIAL FUNCTIONS:


  • Assist the practice manager with clinical staff hiring, training and performance reviews per AMH guidelines
  • Responsible for are all MU, quality measures, PCMH progress to make sure they are turned in and measures met by programs deadline. 
  • NCIR training, vaccine and supply management
  • Assists in evaluation of and developing improved clinical flow processes- QI/QA
  • Be an advocate for team and patients – to provide good patient care and good team morale. 
  • Will provide support for all clinical staff with EMR troubleshooting, template building, EMR utilization to maximize clinic workflow.  


  1. Leadership: Works in partnership with Practice Manager and AXIA to create, support and maintain a culture of professional accountability. Grows and nurtures team members. Creates and maintains a healthy work environments that results in improved patient outcomes, retention, and organizational excellence. Facilitates decision making within the department. Acts with honesty and integrity with team members and patients/families, showing and speaking with respect for the rights of others, human dignity, and diversity. Makes decisions in a timely manner. Coordinates the provision of patient care through delegation, problem solving, time management and organizational Utilizes the ANA Scope and Standards of Practice, Code of Ethics for Nurses with Interpretive Statements, evidence based practice and guidelines from formal nursing organizations as the foundation for nursing practice. Assures that team members provide care that is consistent with regulatory requirements. Promotes collegial relationships with physicians and other professional groups. Removes barriers for staff to assure optimal patient safety and satisfaction. Meets and maintains requirements of a Registered Nurse. Develops and implements plan for self-development in collaboration with the Practice Manager. Holds daily Huddles with clinical support staff.
  2. Interpersonal Effectiveness and Communication: Creates an environment that supports and respects diversity where all team members feel safe, included and free to contribute to the success of AMH. Partners with family and patients to resolve concerns related to the delivery of health care. Communicates with and involves the team through regular dialogue with all team members. Coaches and mentors the team in order to achieve our goals. Provides progress updates to Practice Manager and team members in a timely manner, based on team’s needs. Utilizes communication methods and tools to deliver information that improves quality of personal and organizational decisions. Ensures timely and consistent communication. Interacts with and contributes to the professional development of team members and peers. Inspires team members and helps connect the work of nursing to the goals of the organization.
  3. Business Acumen: Upholds organizational policy and procedure. Provides project/process. Shows imagination through creative solutions. Displays interest and willingness to try new things; takes risks. Monitors operations on a daily basis and provides guidance to Team Leaders. Works with Practice Manager to develop and coordinate unit-based initiatives or programs for a defined population.
  4. Quality Improvement: Working directly with Practice Manager and AXIA, evaluates factors related to safety, outcomes, and effectiveness and develops and implements improvement plans as n Makes decisions based on an understanding of data trending and variation. Embraces change and acts as a change agent. Is a life-long learner. Develops and implements an education plan for areas of responsibility annually in collaboration with Practice Manager. Monitors and improves staff satisfaction through recruitment and retention initiatives.
  5. Performance Improvement: Coach and assist clinical staff and leaders regarding performance Identify barriers, gaps in services and problem-solves solutions to achieve stated goals. Ensure successful implementation and continuous monitoring of performance improvement initiatives. Educate members, providers, vendors/partners, and team members on best practices. Educate providers and other organizational departments in the improvement of member outcomes and satisfaction for assigned population(s). Educate staff, leaders and providers on system wide initiatives – including PCMH, BQPP, payor initiatives, care connections and TCM processes, as applicable. When necessary cross functionality with teams within AXIA to achieve optimal clinic performance. Conduct quality related audits for delegated vendors and provider office visits to facilitate ongoing education and improvement to assigned populations.
  6. Collaboration: Collaborates with Practice Manager and AXIA to identify opportunities for improvement. Assists with promoting collegial relationships with physicians, physician extenders, and other professionals. Assists with promoting collegial relationships with ancillary/support departments within the organization Problem-solving and opportunities to improve. Develops and implements a plan for self-development in collaboration with the Director of Practice Management. Collaborates with the Practice Manager and AXIA in the development of goals, objectives, and action plans to achieve unit/staff education and development. Collaborates with the Practice Manager, Clinicians, AXIA on orientation for new staff.
  7. Human Resources and Organizational Development: Provides ongoing and annual feedback to clinical staff on performance, including performance improvement plans within expected time Coaches staff in developing plan to achieve personal career goals. Facilitates and participates in educating and training staff. Participates in the job posting process, interview process, and supports employees’ equal access to opportunities to develop themselves. Collaborates with Practice Manager to establish a staffing plan. Continually seeks to implement activities that support recruitment and retention. Assures appropriate staffing numbers utilizing HPPD guidelines and considering patient needs/staff skill mix. Assists with compliance with corporate policy.

 

 

 Must be willing to receive all required vaccinations (i.e., flu shot, etc.).
