Description
Are you passionate about providing compassionate care to individuals in need? Do you have a special calling to support patients and families during their end-of-life journey? Hospice of the Piedmont is seeking a dedicated and skilled Licensed Practical Nurse to provide personal care services to Hospice House patients.
This is a PRN role that provides coverage for all or part of the 7pm–7am shift.
Responsibilities:
- Direct Patient Care: Provides nursing care to Hospice residents and oversees nursing assistants.
- Team Collaboration: Works with the team to identify patient care needs.
- Communication: Reports changes in residents' conditions to relevant medical staff.
- Documentation: Maintains accurate patient records.
- Support Tasks: Assists with food preparation, housekeeping, and completes various administrative tasks.
- Policy Adherence: Follows personnel policies and maintains safety and infection control standards.
- Medication Management: Manages medication orders, performs narcotic counts, and ensures proper medication handling.
Qualifications, Skills, and Abilities:
- Licensed as a Licensed Practical Nurse in the Commonwealth of Virginia.
- Two years’ relevant nursing experience required.
- Ability to manage all aspects of terminal care, health team service coordination, and documentation.
- Emotionally mature and caring individual willing to give support and assistance to terminally ill and their families.
- Basic to intermediate proficiency in word processing software.
- Driver’s license, personal automobile, and current automobile liability insurance.

- While performing the duties of the job, the employee is regularly required to use hands to handle, feel objects, tools or controls, reach with hands and arms; stoop, kneel, crouch, or crawl; and talk to hear. The employee frequently is required to stand, sit and climb or balance.
- The employee must occasionally lift and/or move up to 50 pounds and push up to 100 pounds in a wheelchair.
- Noise level is low. Physical and emotional health is necessary for the accomplishment of position duties.
- Due to the nature of services provided, it may be necessary at times for employees to work beyond scheduled work hours to ensure adequate patient care.
