Reports To: Senior Director of ACLT
JOB DESCRIPTION SUMMARY
The bereavement program specialist is responsible for secretarial and program support services for the bereavement program as well as collaborative social service/grief support for palliative services.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
- Assists bereavement director and counselors to maintain quality hospice bereavement and community bereavement programs.
- In electronic medical record software program, enters and discharges bereaved clients and families and runs bereavement reports as appropriate.
- Communicates through telephone calls to bereaved families in a timely manner and documents those calls.
- Distributes letters, information, and literature to families following the death of Hospice of Acadiana, Inc. patients according to the Plan of Care and follows up on returned mailings.
- Assists in preparing, entering, and disseminating bereavement evaluative date.
- Assists in training and managing bereavement volunteers and interns who assist in bereavement programming.
- Provides support with planning and carrying out programs and bereavement events.
- Assists in facilitating and coordinating grief - supportive programs for transitions’ clients and caregivers.
- Assist bereavement director in maintaining monthly metrics / key performance indicators for the bereavement program.
- Participates in hospice activities as required to further the mission of Hospice of Acadiana, Inc.
- Participates in mandatory meetings and functions.
- Accepts and performs other related duties and responsibilities as assigned.
- Provides collaborative, social service/grief support to palliative services.

CONTINUING EDUCATION Requirements
- Attends mandatory hospice educational programs.
Participates in appropriate continuing education as may be requested and/or required by supervisors
Qualifications
- Education: High School Diploma, Associate’s Degree; college preferred.
- Experience: Experience of the grief process and two years clerical experience. Not-for-profit knowledge preferred.
- Special Qualifications: Ability to work as a team member. Understands and accepts hospice mission and issues related to loss and grief. Strong interpersonal and telephone skills. Computer skills. Ability to document client and volunteer interactions as needed.
- Possesses current Louisiana Driver's License, Automobile Liability Coverage.
