Description
SUMMARY
Responsible for providing children, youth, and their families an array of therapeutic services, and working in collaboration with the multi-disciplinary team. In addition to therapeutic interventions, program therapists must engage in case management, ongoing collaboration with outside professionals, and discharge planning for the children/families. Collaborative partnerships with youth and families are critical; Average caseload 12-16 youth/families.
ESSENTIAL DUTIES AND RESPONSIBILITIES (Other duties may be assigned)
Provides general orientation of child/youth and family to FFAST program and treatment options
Completes comprehensive intake assessment that includes mental health, physical health, trauma, and possible substance abuse
Formulates, monitors, and revises a comprehensive client plan, which addresses specific short-term and long-term goals, for the child and family based on assessments and input from the multi-disciplinary treatment team
Assists families in creating crisis/safety plans to prevent/respond to unsafe behaviors or environments that may exist in the home or community and provides crisis phone or in person support 24 hours per day and 7 days per week as assigned
Facilitates a wide array of therapeutic interventions within individual/family therapy based on evidence based practices such as Trauma Focused Cognitive Behavioral Therapy, Motivational Interviewing, and Parent Management training
Maintain accurate, complete, and timely case records in Anasazi per requirements of licensing and private payers, Medi-Cal requirements, and facility policies
Maintain positive relationships with stakeholders, and respond to their needs promptly
Maintain regular communication with FFA social worker (minimum 2 times monthly), County social worker (minimum 1 time monthly), and other important team members (teachers, CASA’s, psychiatrist, etc) to coordinate treatment efforts and connect clients to needed community resources
Follows county guidelines with regard to Pathways to Wellbeing including those related to teaming, collaboration, documentation, use of proper service codes, scheduling of child and family team meetings, and other responsibilities as assigned
Show strong ethical foundation and use System of Care values in the work provided to families (individualized, strength-based, culturally competent, family focused, trauma sensitive)
Fulfills all mandatory training requirements including Cerner, cultural competency, HIPAA, and applicable EBP trainings
Provide training to clinical team or externally in areas of interest/specialization as assigned
Required to be available evenings and occasional weekends as assigned by supervisor or as client needs dictate
Other duties or projects as assigned

Qualifications
MINIMUM REQUIREMENTS
- Master’s Degree in Social Work, Marriage and Family Therapy, Psychology, Counseling, or related field from an accredited university
- Must be a registered intern in social work (ASW), psychology (Ph.D/Psy.D.), or marriage and family therapy (IMFT), or licensed LCSW, MFT, or Psychologist
- Valid California Driver’s License, acceptable diving record, reliable transportation, and vehicle insurance
PREFERRED QUALIFICATIONS
- Knowledge of trauma, attachment, grief/loss, and foster care culture are a plus
- Experience with Parent Management Training, Positive Discipline, Trauma Focused Cognitive Behavioral Therapy, PCIT PRIDE Skills, Motivational Interviewing
- Training/experience in the area of children and families
- Knowledge of basic case management skills
- Ability to work effectively in a multidisciplinary setting
- Proficient in Cerner (electronic medical records system)
- Proficient in Microsoft
