- Deliver trauma informed, culturally and linguistically competent care that supports recovery
- Conduct Child and Adolescent Needs and Strengths (CANS) Assessments, and Stanley Brown Safety Plans
- Provide crisis intervention, as needed, including participating in a weekly on-call rotation.
- Maintain up-to-date documentation for all cases
- Participate in daily team meetings
- Communicate with schools, medical personnel, and other treatment adjacent providers, as appropriate.
- Follow all established organizational and oversight policies, procedures, and guidelines.
- Must have a Bachelor’s Degree and at least one year of relevant experience
- Must have a valid Driver’s License, reliable transportation, and be willing to transport clients in own vehicle
- Will have computer literacy skills, including use of Microsoft Office Suite and an ability to learn/utilize an Electronic Health Record
- Must be able to work independently and have good organization, writing, and telephone skills
- Must be flexible, and able to prioritize tasks
- Must be able to work as a team player with colleagues and other providers
- Will be willing to participate in ongoing training
- Will have experience providing case management, including assisting with applications for Medicaid, Social Security, SNAP Benefits, etc.; preferred
- Is bilingual in the languages of English/Spanish; preferred
