RN Case Manager in Calhoun County and surrounding areas

Big Bend Hospice (BBH) is your “Hometown Hospice” providing compassionate end-of-life care for over 40 years to the Big Bend region. Guided by our values of integrity, respect, and excellence, we are seeking an experienced Registered Nurse (RN) Case Manager join our team in Calhoun County and surrounding areas.  

 

 

The ideal candidate will be exceptional in supporting a team of clinicians in delivering comprehensive Hospice and palliative care services. As a Case Manager, you will identify and coordinate the needs of chronically ill patients and families. Also, you will work collaboratively on an interdisciplinary team (IDT) to help team members operate as efficiently as possible.

 


Position Duties and Responsibilities:  
The Registered Nurse (RN) Case Manager responsibilities will include, but are not limited to the following: 

  • Supervises Hospice Aides and Licensed Practical Nurses and initiates their Plans of Care.
  • Evaluates patients and families for admission using approved hospice criteria and procedures.
  • Conducts pre-assessment and admission visits in home, hospital, or facility settings.
  • Coordinates with physicians, intake teams, and community partners to ensure timely hospice admissions.
  • Educates patients, families, and referral sources on hospice philosophy, eligibility, and available services.
  • Ensures admission documentation is accurate, complete, and submitted within required timeframes.
  • Develops and implements individualized interdisciplinary group (IDG) plans of care for patients and families.
  • Provides skilled nursing care, symptom management, and patient/family education based on hospice best practices.
  • Maintains consistent communication with physicians, team members, and community partners for continuity of care.
  • Participates in interdisciplinary team meetings, quality assurance, and continuous improvement activities.
  • Demonstrates expertise in pain management, symptom control, end-of-life care, and holistic practices.
  • Ensures documentation in the electronic medical record is accurate, timely, and compliant.
  • Maintains knowledge of Medicare, Medicaid, and other hospice regulatory requirements.
  • Adheres to all Big Bend Hospice, state, and federal policies and laws governing hospice practice.
  • Serves as a clinical resource and mentor to colleagues, new employees, and students.
  • Represents Big Bend Hospice with professionalism and compassion in all interactions.
  • Collaborates with Admissions, Education, and Community Relations teams to ensure smooth patient transitions.
  • Participates in assigned weekend, evening, and holiday rotations.
  • Manages a balanced caseload consistent with organizational standards.
  • Demonstrates accountability, sound clinical judgment, and strong prioritization skills.
  • Practices responsible and efficient use of organizational resources and supplies.
  • Performs other duties as assigned.

Work Schedule:  

  • Monday-Friday; 8:00 am- 5:00 pm; and holiday rotations

Travel Requirement: You must have a reliable vehicle and be willing to be dispatched throughout our service areas.

 This position will cover the following counties: 

  • Gadsden
  • Liberty
  • Calhoun
  • Jackson
  • Gulf

Skills and Qualifications


Education & Experience: 

  • Must have an active Florida RN license in good standing
  • BSN is preferred
  • Minimum of 2 years of nursing experience required
  • Hospice experience is a plus
  • EMR experience required

  • Able to manage the physical demands and emotional stress of work in a field position, caring for terminally ill patients and their families.
  • Must possess a valid Florida driver's License, valid auto insurance, and reliable transportation.
