Pay Range from $84,000 to $110,000

We are seeking an experienced and motivated Regional MDS Nurse (LPN or RN) to support our Director of MDS and ensure accuracy, compliance, and quality across multiple skilled nursing facilities. This key role ensures our residents receive the highest standard of care while optimizing the reimbursement process and strengthening documentation practices.

Key Responsibilities:
Provide ongoing support to facility MDS coordinators across multiple sites.
Conduct on-site visits to assess, train, and guide facility teams.
Ensure MDS assessments are completed accurately and in compliance with federal and state regulations.
Review and analyze data to identify opportunities for improved documentation and reimbursement accuracy.
Participate in audits and assist in developing corrective action plans as needed.
Offer educational guidance and mentorship to strengthen MDS understanding at the facility level.
Collaborate with interdisciplinary teams to enhance resident outcomes and promote operational excellence.
Qualifications:
Current NYS LPN or RN license required.
Minimum 2 years of MDS 3.0 experience preferred.
Strong knowledge of Medicare and Medicaid reimbursement systems.
Excellent communication, leadership, and problem-solving skills.
Self-motivated with strong attention to detail and a commitment to teamwork.
Candidates may be based in the Poughkeepsie, Highland, Binghamton, Syracuse, or Corning areas.
Must be willing and able to commute to facilities throughout the Hudson River and Central New York region.
Valid NYS driver’s license and reliable transportation required.
