Position Overview
The LPN Nurse Coordinator plays a vital role in supporting the health and well-being of the individuals we serve. This position provides direct nursing care, oversees medical documentation and communication, and supports the Health Services Director in managing day-to-day nursing operations. The Nurse Coordinator ensures our clients receive quality medical attention and compassionate care in alignment with our mission and standards.
Key Responsibilities
Review and audit Medication Administration Records (MARs) to ensure accuracy and compliance.
Communicate and monitor new medical orders, ensuring timely and proper documentation in the software system.
Coordinate medication management, ensuring all clients have necessary prescriptions and refills.
Train and mentor new nursing staff under the direction of the Health Services Director or RN Supervisor.
Participate in Individual Support Plan (ISP) meetings as assigned, coordinating medical information and updates.
Assist with on-call responsibilities as needed.
Maintain up-to-date client vitals and medical documentation in the system.
Review daily nursing notes to ensure thorough and accurate documentation.
Obtain, file, and track medical reports and provider communications.
Support the maintenance of accurate and complete medical records.
Administer medications and treatments as prescribed, and assist providers with examinations as required.
Notify the RN Supervisor, Health Services Director, and/or physician of any client health concerns.
Ensure nursing staff are trained and knowledgeable about each client’s health and behavioral support plans.
Conduct LPN performance reviews and assist in corrective action processes with approval from the Health Services Director.
Assist with scheduling, shift coverage, and overall team coordination.
Provide ongoing education to staff regarding health concerns, procedures, and policies.
Manage and track medical supply inventory and assist with ordering as needed.
Attend all required nursing and interdisciplinary team meetings.
Perform additional duties as assigned to support the department’s success.
Qualifications
High School Diploma or GED required.
Graduate of an accredited Licensed Practical Nurse (LPN) program.
Current and active Tennessee LPN license in good standing.
Current CPR certification.
Valid driver’s license with acceptable driving record.
Ability to lift, reposition, and support up to 50 pounds as part of direct client care.
