Tri County Care Management Organization (TCCMO) is a non-profit organization that provides care management (case management) services for children with complex challenges in Hunterdon, Somerset and Warren Counties.
We are currently seeking a Care Manager Supervisor based in our Branchburg, NJ office to provide leadership and support to Care Management staff. This is a full-time position.
Our Care Manager Supervisors play a pivotal role in ensuring Care Management staff have the tools they need to provide youth and their families the best possible care through intensive wraparound services that help youth succeed at home, in school, and in the community.
Responsibilities of a Care Manager Supervisor:
- Supervise team of Care Managers and oversee operations of the team to ensure compliance with standards of care
- Utilize Quality Assurance tools to adhere to best practice standards and policies
- Lead team meetings and provide regular supervision to Care Managers
- Assist Care Managers in identifying and formulating strategies to overcome barriers to service delivery
- Collaborate with Care Managers on all aspects of Care Manager duties, including leading and scheduling Family Team meetings, preparation of crisis plans and Individualized Service Plans (ISP), and support in the field during meetings
- Recommend community support and services for youth and families and collaborate on authorization for provider services as needed
- Monitor the entry of information in electronic health records
- Actively participate in formal and informal quality review efforts
- Participate in after hours and weekend on-call Supervisor rotation to provide back up support to the on-call Care Manager
Qualifications
Position Requirements:
- Master’s degree in social work, psychology or related discipline
- At least five (5) years of clinical experience in a child service system environment
- LCSW or LPC license preferred
- Previous supervisory experience preferred
- Strong problem solving and interpersonal communication skills
- Proficiency with MS Office Suite and related technology
