Position Summary


The Care Coordinator will work in support of the care team through collaboration with other team members and providers to ensure seamless treatment management for clients enrolled in outpatient services.

Primary Functions

1. 1. Facilitate appointments for Just in Time scheduling

2. Maintains communication between care team and providers involved with client care

3. Provides linkages to internal and external community resources for new and established patients

4. Serves as a liaison between community agencies and behavioral health by coordinating care

5. Conducts record reviews for presence of pertinent funder driven documentation

6. Relays relevant information to team members across systems (i.e. upcoming appointments, multiple
same day appointments, needed case management support, hospital discharges, etc.)

7. Coordinates the discharge planning of in-patient clients

8. Encourages and promotes the use of Health Information Technology resources

9. Documents within EMR in a manner most beneficial to the care team for wraparound support

10. Coordinates with the care team for appropriate referrals for specialty care

11. Completes all reports in a timely manner

12. Uses protocols and policies and procedures to guide care

13. Completes audits for quality management

14. Approaches problem-solving through a patient-centered lens

15. Attends all required training and continuing education as required or provided.

16. Other duties and/or responsibilities as assigned

17. Completion of required forms per guidelines or protocols

18. Ability to work independently and manage multiple responsibilities appropriately

19. Ability to document in a concise and professional manner

Educational Requirements

BA, BS Degree or Higher in Health and Human Services, Psychology, or related field

Experience & Qualifications

1. BA, BS Degree or Higher

2. 3 to 5 years related experience working with behavioral health clients preferred
3. Strong organizational and time management skills
4. Skilled in Word, Excel, and Outlook
5. Strong documentation skills
6. Strong verbal communication skills
7. Exhibits the ability to work effectively with people (staff, visitors, and clients).
