About the Opportunity:

Schedule: Monday-Friday 8:30am-4:30pm

Location:  Keizer, Oregon

Starting Pay:  $25.79/hour (depending on experience)

Job Summary:

Works as a critical part of the Day Treatment Team to ensure consistent and supportive coordination is provided between local school districts and the Day Treatment Program.  Supports local schools in helping them prepare for a client’s return from Day Treatment into their local school with respect to a client’s educational, mental, and behavioral health needs and strengths.  Supports Day Treatment therapists with discharge planning related to mental health, physical health, and educational needs.  Works in collaboration with CFT to make referrals and schedule appointments for psychiatrist, therapists, and applicable mental health resources for clients' transition into a lower level of care."

About the position: 

  • Coordination with home School District for transportation purposes at the time of admit.
  • Within the first week of a client’s admission begins coordination with the local school district to identify discharge school placement options.
  • Acts as a liaison between the Day Treatment Program and the local school districts, building relationships with local districts to support discharge planning.
  • Works in tandem with schools to support a client’s transition back to the local school with respect to both educational and mental health issues.
  • Ensures school districts are represented in the treatment planning process and in any ongoing treatment team meetings, providing input into the clinical assessment process. 
  • Consults directly with classroom teachers and school administration to implement any behavioral or educational planning that could be helpful to the school in supporting the client’s transition back to school. 
  • Starting at intake, works closely with the Day Treatment Therapist to ensure adequate mental health services are set up for the client to transition into at discharge.
  • Ensures the new school program is connected to the clients discharge mental health provider to support the clients’ successful discharge.
  • Promotes the Day Treatment Program and the short-term stabilization model with local school districts as well as with Medicaid and Private Insurance funders.
  • Consult with therapists, psychologists, educators, and other treatment team members regarding treatment issues and concerns; as well as discharge planning

About You:  

TOP 3 MUST HAVES:

  • Demonstrates an understanding of child and adolescent development and treatment standards, models, concepts, and theories within a multidisciplinary setting.
  • Demonstrate ability to professionally represent agency and support coordination with external agencies serving youth, coordinate care for transition and community supports. 
  • Understand, lead, and participate in treatment process including interdisciplinary communication, engagement, documentation, and coordination of care responsibilities. Ability to administer medications to clients when needed, per policy.

PREFERENCE (but not a must)

  • Education: Bachelor’s degree in behavioral sciences or equivalent education/experience.
  • Experience: Two (2 years) full years experiences as a direct care worker with at-risk children and/or adolescents is preferred.
