Description of Primary Responsibilities
- Effectively communicates with participants, families, staff, and other agencies and departments.
- Drives a specially equipped van to deliver physically limited persons to varied destinations. Assures that wheelchair lifts and tie-down equipment are properly operated and that participants are delivered to appointments in a timely manner.
- Follows established schedules, adjusts schedules to meet unanticipated changes, and determines the safest and most efficient pick-up and delivery route.
- Assists participants on and off the van in a safe and courteous manner. Deals with medical equipment and other emergencies that may arise.
- Inspect the van and maintain the vehicle in a clean, orderly, and safe condition. Reports on the need for maintenance.
- Assure all necessary program records are maintained according to policy.
- Observe each participant for any change in physical, mental, emotional, and social functioning and shall report such changes to the licensed nurse.
- Completes documentation as required.
- Assists in the coordination of 24-hour care delivery with assistance from the FHCN Navigation Center.
- Other projects and duties as assigned.
Description of Primary Attributes
Professional & Technical Knowledge:
- Must possess a high school diploma or General Educational Development (GED) certificate and be at least 21 years old.
- Minimum 1-year experience transporting individuals with special mobility needs.
- Interact in a friendly and professional manner with participants and their families.
- Communicate effectively and openly with other Interdisciplinary Team members on patient status and needs.
- Only act within the scope of his/her authority to practice.
- Meet a standardized set of competencies for the specific position description established by FHCN PACE and approved by regulatory agencies before working independently.
- Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
- Pass a criminal background which will verify that the candidate is: not excluded from participation in Medicare and Medicaid programs; has not been convicted of criminal offenses related to their involvement in Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act; and have not been convicted of physical, sexual, drug or alcohol abuse.
Technical Skills:
- Ability to prepare basic correspondence and simple reports in Microsoft Word.
- Ability to use Microsoft Excel to create tables and simple displays of information.
- Ability to create basic presentations in Microsoft PowerPoint.
Licenses & Certifications:
- A Valid Driver’s license and an acceptable driving record are required.
- Upon employment, a defensive driving course, first aid, and CPR certifications are required to be kept up to date.
- Currently certified in cardiopulmonary resuscitation (CPR), if CPR is not current, will be granted 30 days from the date of hire to schedule for the next available CPR certification class provided by FHCN.

Physical Demands:

- Physical demands: Employees may be required to conduct home visits to the private homes of FHCN PACE participants for transportation pick-ups. On occasion, when fulfilling Driver duties, the Employee will assist elderly patients during transit to and from the Adult Day Health Center. When acting as a Driver, the Employee will have significant hands-on care of participants and the usual and customary methods of performing the job’s functions require the following physical demands to assist participants with ambulation: significant lifting, carrying, pushing, and/or pulling, stooping, grasping, kneeling, crouching. There will be repetitive motion of the wrists and hands. Employees must be able to stand, bend, and walk for extended periods of time. As a Driver, an Employee must be able to sit and remain alert for extended periods of time while operating a vehicle. Employees are required to use arms and legs during transit escort. Employees may also, on occasion, have to assist with lifting participants and help them turn, walk, or stand. Ability to lift/move up to 75 lbs occasionally, and up to 50 lbs frequently.
- Work Environment: Employees must be able to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage the FHCN PACE members’ care. On occasion, the Employee will spend time in a vehicle transporting participants. The primary work setting is in an Adult Day Health Center and primary care clinic environment with moderate noise levels and controlled office temperatures.
