We are seeking a dedicated and compassionate Social Worker (SW) to join our Home-Based Palliative Care Program – Living Well New York, supporting patients and families living with serious, chronic, or life-limiting illnesses. This role involves providing care in patient homes and working closely with an interdisciplinary team to deliver comprehensive, person-centered support that improves quality of life and honors patient values. The SW will play a key role in addressing the emotional, social, spiritual, and practical needs of patients and caregivers, guiding them through complex medical decisions and care transitions.

The salary range for this position is $60,000k - $75,000k annually and is commensurate with education, experience and hospice certifications. This is a full-time benefits eligible position. Schedule: Monday through Friday, 8:30 AM to 4:30 PM. This includes a shared weekend rotation on a monthly basis

Responsibilities

    Conduct assessments for patients receiving palliative care at home, identifying strengths, needs, risks, and goals.

    Provide emotional support to patients and families coping with serious illness, grief, and end-of-life issues.

    Facilitate discussions around goals of care, advance directives, and end-of-life planning.

    Assist patients and caregivers with navigating healthcare systems, including referrals to community resources, financial assistance, long-term care, and home support services.

    Participate in interdisciplinary team meetings, contributing to holistic care planning alongside physicians, nurses, and chaplains.

    Advocate for patient and family preferences, ensuring care aligns with their values, culture, and wishes.

    Provide crisis intervention and support during acute changes in condition or caregiving challenges.

    Maintain thorough and timely documentation in the electronic medical record (EMR) in compliance with organizational policies and regulatory standards.


Qualifications

    Master's degree in Social Work (MSW) from an accredited program, NYS LMSW required, NYS LCSW preferred.

    Experience in palliative care, hospice, home care, or medical social work, preferred but not required.

    Demonstrated knowledge of serious illness, grief, bereavement, family dynamics, and community resources.

    Strong communication, counseling, and clinical documentation skills.

    Ability to work independently in the community and collaboratively within an interdisciplinary team.

    Valid driver’s license and reliable transportation for home visits.


Preferred Qualifications

    Certification in Hospice and Palliative Care Social Work or willingness to obtain.

    Bilingual or multilingual abilities a plus.

    Experience in advance care planning and facilitating family meetings.

    Community-based role involving home visits and in-person meetings at healthcare partner locations.
