Description
The Intake Manager is responsible for managing all aspects of the patient intake process to ensure hospice admissions are handled in an organized and timely manner. This includes coordinating the insurance verification process and maintaining active communication with all participants during the admissions process.
RESPONSIBILITIES:
- Manages the intake process for hospice: sets up patient account information, collects all data from family members and physicians and inputs information into Electronic Medical Record (EMR), ensuring employee assignments for access to the information
- Initiates pre-admission criteria
- Ensures that admissions are processed and completed in a safe and timely manner to meet internal criteria and patients' needs
- Reviews and updates referral information as needed to assure accuracy and completeness
- Manages all assigned intake/admission EMR workflow for hospice patients (initial referrals, physician verification, etc.) per assigned care team
- Ensures receipt of all insurance information on intake; negotiates with insurance companies when needed for reimbursement; obtains initial payer source eligibility verification and coordinates necessary follow-up to communicate patient’s expected out-of-pocket expense
- Checks the Medicare Web Portal and prints confirmation prior to patient admission; sends notification to appropriate parties of prior hospice episodes
- Provides team with daily reports regarding pending admissions, new admissions and any outstanding (open) items
- Acquires, maintains and uploads signed travel contracts into EMR
- Responsible for answering phones along with other designated staff
- Documents referral activities and communicates information to appropriate staff
- Responsible for the initial patient chart formation and the dismantling of the chart after death/discharge
- Coordinates and facilitates timely face-to-face visits with Nurse Practitioner under Medicare guidelines
- Maintains and updates list of referrals and pending admissions to assure accuracy at all times
- Acquires, maintains and files signed contracts with all Skilled Nursing Facilities and individual patient contracts within those SNFs
- Enters physicians and organizations into EMR
- Contacts referring physician to obtain signatures on initial orders and gathers relevant medical records from referring physician and from attending physicians when necessary
- Collaborates with Clinical Managers to ensure efficient, timely scheduling of evaluations and admissions
- Orders and faxes confirmation of all Durable Medical Equipment for patients on admission
- Maintains confidentiality with regard to patient, staff and volunteer information in accordance with company policy
- Conducts or assists with special projects as requested
- Works as a member of the Quality Assurance Performance Improvement (QAPI) team to ensure program compliance with Conditions of Participation (COP) and ensure ongoing quality performance monitoring
- Initiates and maintains a liaison relationship with healthcare providers to effectively coordinate or follow Clinical Manager directives for multidisciplinary care; reliably communicates pertinent information to appropriate members of the interdisciplinary team
- Performs other duties as directed
Requirements
- Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing; RN license preferred
- Minimum of one (1) year experience in hospice/home care preferred
- Knowledge of state, federal and local regulatory requirements preferred
- Excellent interpersonal, written and verbal communication skills
- Ability to work cooperatively with all members of the organization
- Demonstrated ability to work as a member of a team, with strong organizational skills
- Prior use of Electronic Medical Records (EMR) preferred
- Must be able to prioritize duties daily in response to rapidly changing patient care needs; ability to utilize outstanding customer service skills
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order; ability to travel to all business locations for training and/or meetings
