Hiring For Client Care Coordinator in the Palm Desert area.

Pay ranges between $21-23 an hour

  • JOB SUMMARY

    The Client Care is a central point for efficient home care service delivery. This role involves managing inbound calls, maintaining staff information, and providing staffing solutions for new client referrals. As the Client Care Coordinator, you will work closely with the Management team to execute organization goals and meet client needs, while supporting other internal departments.

    DUTIES & RESPONSIBILITIES

    1. Sign on clients and explain the service agreement and details to them

    2. Perform initial client home assessments and create a documented plan of care

    3. Perform routine assessments to monitor client's condition and home environment

    4. Review care plans with caregivers and families to ensure all needs are met

    5. Establish initial client records and maintain the EHR database accurately, keeping the client chart and information up to date for compliance

    6. Start cases and fill in on the field, traveling to ensure services are provided to the clients as needed and when emergencies arise

    7. Attend client care meetings, staff meetings, and educational sessions , staying up to date with training

    8. Supervise and train personnel to promote staff growth and skill development so as to improve service quality, ensure adherence to the plan of care, and meet client needs

    9. Maintain proper communication between clients, personnel, and the agency while documenting all call information

    10. Resolve client complaints, escalating when necessary, and obtain client satisfaction surveys

    11.Develop relationships with referral sources (phone, email, in-person, networking)

    12. Participate in the after hours on call rotation (including weekends, nights, and holidays)

    13. Assist in scheduling personnel appropriately to fit client needs

    14. Perform other administrative tasks as assigned

    15. Maintain documentation of all tasks according to policy and adhere to compliance standards

    16. Evaluate caregiving and related services to continuously improve care quality, staff, efficiency, and the home care program

    JOB REQUIREMENTS

    • Demonstrates strong written and oral communication skills in person, over the phone (verbal and text) and over email.

    • Establishes productive organizational skills and habits.

    • Provides excellent customer service skills.

    • Strong  team skills.

    • Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.

    • Technical aptitude to know, learn and master the agency’s software.

    • Ability to multi-task in a fast paced office environment.

    • Must have and maintain an HCA registration in good standing

    • Must have valid I9 Employment Eligibility Verification documents

    • Must have and maintain a valid CA driver's license and a good driving record
