Your role at BHSN:

As a Psychosocial Rehabilitation Practitioner at BHSN, you will deliver psychosocial services in order to rehabilitate and support service recipient's developmental functions. 

Schedule: Monday - Friday 11:00am - 7:00pm with some flexibility

What your day might look like:

  • Provides individual and/or group Psychosocial Rehabilitation Services as permitted by credentials, scope of practice, and training, to an assigned caseload 
  • Provides supportive services in the home of the child/family, in the community, or at agency program sites, as appropriate in order to most effectively restore, rehabilitate, and support the child/youths developmental functioning 
  • Develops, documents, and provides a variety of age appropriate skill building activities in accordance with corresponding Medical Necessity guidelines 
  • Participates in the writing and timely completion of service plans, progress notes, safety plans and other required documentation in accordance with State/federal regulations, program requirements and BHSN policies and procedures 
  • Develops and documents measures to track progress toward service plan objectives 
  • Maintains direct service productivity target as established by the agency; Productivity targets are subject to change depending on agency needs 
  • Provides information and education on CFTSS/SPA and HCBS Waiver services to clients, families, other programs or community partners to support informed decision making and enhance community awareness of program services 
  • Successfully completes all required trainings as well as those recommended to improve service delivery to otherwise enhance job performance 
  • Meets or exceeds any monthly contact requirements as defined by regulations and/or caseload sizes 
  • Attends as a participatory member of department meetings and trainings as required 
  • Performs other duties as assigned within context of job description 

Your skills and qualifications:

  • HS diploma or equivalency preferred OR a State Education Commencement Credential with a minimum of three years' experience in children's mental health, addiction and/or foster care; OR a Bachelor's degree in social work, psychology, or in a related human services field; OR a Master's degree in social work, psychology, or in a related human services field 
  • Ability to work with children in home and community-based settings 
  • Completion of Mandated Reporter training 
  • Strong engagement, organizational, and communication skills required 
  • Ability to work a flexible schedule to meet the needs of the families being served 
  • Ability to work collaboratively with other community services and providers 
  • Valid NYS driver’s license with a driving record acceptable to BHSN insurance carrier and reliable transportation required for local travel
