About the Department

Brunswick County is currently accepting applications for a Case Manager to join our Emergency Medical Services team and support our Community Paramedic and Access and Functional Needs (CP-AFN)  programs.  At Brunswick County we’re committed to providing highly responsive, effective, and efficient emergency medical and planning services for citizens and visitors across Brunswick County and surrounding areas.  The CP-AFN Case Manager plays a vital role in ensuring we’re able to positively impact the health, safety, and welfare of County residents during times of crisis, natural disasters, and other emergencies.  

The CP-AFN Case Manager will provide case management support to individuals participating in the Community Paramedic program.  The focus of this work will be to connect clients with resources and services to assist them to remain in their homes and address physical and mental health needs, including substance use.  The CP-AFN Case Manager works in collaboration with other team members and the community to assess and link individuals to programs and services.  Responsibilities include:

  • facilitating the needs and care options for community paramedic patients.
  • serving as a liaison between community paramedics and providers.
  • monitoring patient progress.
  • identifying and linking patients to services.
  • advocating for and educating individuals and family members.

The CP-AFN Case Manager will also be responsible for maintaining and coordinating the County’s Access and Functional Needs Program.  Work involves planning, developing, implementing, and managing countywide program activities before, during, and after disasters to integrate a whole community inclusion approach.

Work will require the ability to work independently and successfully balance time spent supporting clients and meeting the documentation requirements of the position. This position reports directly to the Director of Emergency Medical Services. 

Minimum Qualifications

Bachelor's degree in social work from an appropriately accredited institution; OR bachelor's degree in a human services field from an appropriately accredited institution and one (1) year of directly related experience; OR bachelor's degree from an appropriately accredited institution and two (2) years directly related experience; OR an equivalent combination of education and experience which provides the required knowledge, skills, and abilities to perform the essential functions of the position.  

  • Valid NC or SC Driver License required.
  • Proficient in using Microsoft Office Suite to include Word and Excel.

Physical Requirements
Must be physically able to operate a variety of automated office machines including computers, calculators, copiers, facsimile machines, etc.  Must be able to exert up to 10 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects.  Sedentary work involves sitting most of the time but may involve walking or standing for periods of time.
