JOB SUMMARY
The RN Admission nurse is responsible for coordination the transition of new and existing patients back
to, or into, the Inpatient Hospice Hospital or Home Care setting for hospice or palliative services. This
position is a “hands on” position. The RN Admission Nurse must support the Connecticut Hospices’
vision, mission, and operating principles.

ESSENTIAL DUTIES AND RESPONSIBILITIES
General:
• Completes the admission evaluation initial and comprehensive assessment
• Completes admission required documents of the election of the benefit, consents, creating the plan
of care.
• Documents all information for care coordination into the electronic medical record system (NDoc).
• Provides education specific to the patient’s diagnosis, reason for admission, and goals.
• Functions as resource person to community agencies, local hospital care coordinators, discharge
planners, social workers, physicians, nurses, patients, families, and other community organizations.
• Works closely with the Connecticut Hospice Liaison RNs to coordinate transfers, provide guidance
and direction, and coordinate education for patients and families.
• Work closely with Nursing, and Medical departments to review potential admissions.
• In absence of Department Director, may manage, review, and process information and clinical
services.
• Provide coverage within the Centralized Intake and Admissions (CIA) Department.
• Alerts nursing, social work, and Inter-Disciplinary Team (IDT) to specific needs and/or concerns.
• Perform duties at either the CIA office or in the field as a Liaison RN at specific facilities.
• Participate in orientation of new employees, staff education, family meetings, patient, family,
community, and peer education.
• Assist in meeting all regulatory requirements including the Joint Commission, State licensure, and
OSHA.
• Must understand and follow company policies including harassment and compliance procedures.
• Displace integrity and professionalism by adhering to the Connecticut Hospice Code of Conduct.
• Complete mandatory compliance trainings.
• Work as directed by supervisor• Communicates with the interdisciplinary team and designated physician
• Provides consultation in hospitals when there is a patient in a contracted bed
• Accepts assignments of the CIA Director
• Provides supportive documentation to the referral source, skilled nursing facilities.
Health & Safety:
• Report and take required action regarding injuries, incidents, medical, and accidents required by
policy.
• Make the health, safety, protection of rights, and the development of all employees your primary
responsibilities.
• Effectively manage medical and first aid emergencies.
• Follow all agency safety/medical related policies and procedures.
• Follow fire evacuation and disaster procedures correctly and in a timely manner.
• Report unsafe situations immediately; ensuring safety is imperative.
• Ensure pertinent emergency telephone numbers are readily available.
• Report symptoms and signs of communicable diseases immediately.
• Consistently use universal precautions.
• Report any incident that could potentially result in negative reactions from the community, law
enforcement officers, and other agencies to the supervisor immediately.
Qualifications
EDUCATIONAL REQUIREMENTS, QUALIFICATIONS, and TRAININGS
• Must have active, and in good standing, Registered Nurse (RN) license in the State of Connecticut.
• Degree of Nursing from accredited school of nursing.
• Hospice and palliative home care nursing experience.
• At least five years-experience in clinical healthcare role.
• Knowledge of current nursing, medical and health trends, and practices in rendering of patient care.
• Knowledge of referral procedures, criteria, and admission guidelines.
• Must have excellent communication skills and speak in public.
• Must pay attention to details.
• Capacity to maintain accurate records and exercise discretion in handling of confidential
information.
• Willingness to work a flexible schedule; including at different sites and programs as needed.
• Ability to maintain assigned work hours.
COMPUTER and SOFTWARE REQUIREMENTS
• Computer literate and proficient in Microsoft Office Word and Excel, Outlook, plus word processing
and presentation software.
• Ability to operate media equipment such as tablets, smartphones, and other electronic equipment.
• Ability to work with general office equipment
• Ability to work with and understand databases a must and the ability to learn technical skills.
PHYSICAL REQUIREMENTS and WORK ENVIRONMENT
• Shall have essential physical skills; be able to run, squat, stoop/bend, kneel, climb stairs, be able to
lift at least 50 pounds (or ¼ own body weight), push/pull against potential resistance, and be able
to implement non-physical and physical interventions and CPR with individuals supported when
needed.
• Shall have adequate vision, reading, writing, and documentation skills, and hearing to perform the
essential functions of the job.
• Must have enough endurance to perform tasks over long periods of time.
• Will be engaged in speaking, sitting, walking, driving, listening, and in communicating both orally
and in writing while performing his or her duties.
• Must be able to listen and respond to questions and instructions.
