POSITION
Part-Time 10-29 hours a week; Variable schedule based on number of residents each week.
GENERAL DESCRIPTION
The Anti-Trafficking Case Manager facilitates the care of our safe house residents by providing case management services and connecting them with community resources and service providers. The Case Manager works with each resident to develop, implement and monitor individual case plans with the goal of effective progress toward the next step in the continum of care. The Case Manager facilitates effective communication and collaboration with all invested parties.
DUTIES AND RESPONSIBILITIES
Case Management
- Assist in intake of new residents by reviewing referral records and applications
- Meet with each new intake to identify her case management needs and begin to set expectations for the services she will receive, which may include obtaining identification and benefit documentation, medical/mental health assessments and appointments, legal services coordination, etc.
- Develop individual service plans by assessing treatment needs; develop and evaluate treatments plans progress; facilitate interdisciplinary approaches, monitor resident progress in weekly meetings on the status of case management actions
- Implement treatment programs by recommending daily schedules and routines; coordinating services being provided, arranging resources, including transportation and escort as needed
- Participate in organizational care team meetings to assess each resident’s progress and plan for her needs
- Empower residents to progressively meet their needs independently
- Assist in residents’ discharge by reviewing and documenting discharge plans; coordinating discharge and post-discharge requirements; providing resources
Reporting
- Maintain residents’ records by reviewing case notes; logging service plans, service provision, incidents and progress
- Maintain reporting relationships with parole/probation or other legal providers
- Prepare a comprehensive package of the residents’ personal papers that will be given to her upon her departure from the program
Collaboration
- Draft MOUs with preferred providers and present draft to Executive Team for approval prior to implementation
- Develop NCMS database system to support a wide range of clients’ needs and community resources
- Continually assess the efficacy and value of external providers and make appropriate recommendations if change is needed
- Interact with residents and staff through activities and class instruction
- Engage with Care Team in a supportive and respectful manner
- Serve as a credible and courteous ambassador of the Raleigh Dream Center particularly to external service providers, residents and team members
Qualifications
- Bachelor’s degree in human services, psychology, social work, counseling or related field
- Two years of case management experience coordinating and documenting client service provision with knowledge of current case management practices
- Knowledge of/experience with trauma informed therapeutic approaches and environment
- Familiarity with the needs of individuals who have experienced commercial sexual exploitation
- Experience working in a residential facility preferred
- Experience with conflict resolution and crisis management
- Proficient in computer use and case management software
- Excellent time-management skills
- Excellent oral and written communication skills
Requisites
- Must have a current, valid driver’s license
- Successfully pass background checks and random drug testing
- CPR and First-Aid certification preferred or be willing to obtain training after hire
- Must be willing to complete job specific training pertaining to Human Trafficking, including Trauma Informed Care, Case Management and the Triune Care Model after hire
