Adult Transitions Registered Nurse (RN) - Travel Position  
Location: Kansas City, MO

Pay: $1936.80/week  
Completion Bonus: Up to $7035 total (You will receive $2345 after the successful completion of each of your first 3 contracts with us)

Job Description:  

We are seeking a highly organized and experienced Adult Transitions Registered Nurse (RN) to manage and coordinate the care of patients moving through different phases of treatment, such as from critical care to a general unit, or from hospital to post-acute care. This specialized role is critical in preventing gaps in care, optimizing patient throughput, and reducing hospital readmissions. The Transitions RN acts as a vital bridge, ensuring effective communication, comprehensive patient education, and seamless handoffs. The ideal candidate is an excellent problem-solver, possesses strong clinical acumen across various specialties, and is a dedicated patient advocate.

Key Responsibilities:  

• Coordinate and facilitate the safe and effective transfer of patients between units (e.g., ICU to Med-Surg) and external facilities (e.g., hospital to SNF or home health).
• Conduct thorough patient assessments focused on identifying barriers to transition, such as medication issues, follow-up needs, or home safety concerns.
• Ensure all necessary documentation, including discharge summaries and transfer orders, is complete and accurate prior to patient movement.
• Provide intensive patient and family education regarding new medications, warning signs, and necessary follow-up appointments.
• Collaborate with case management, physicians, therapists, and community resources to arrange post-discharge services.
• Monitor and analyze transition metrics to identify opportunities for process improvement and reduce avoidable readmissions.
• Act as a clinical resource to staff regarding patient readiness for transfer.

Qualifications:  

• Active RN license in Missouri.
• Associate’s or Bachelor’s Degree in Nursing.
• Basic Life Support (BLS) certification.
• Minimum of 1-2 years of recent clinical nursing experience (Med-Surg, Telemetry, or ICU preferred).
• Prior experience in Case Management, Care Coordination, or Discharge Planning is highly preferred.
• COVID vaccine and recent TB test may be required.
